腕踝针联合二陈汤对肺癌根治术后患者免疫功能和炎症因子水平的影响

Effect of Carpus-Ankle Acupuncture Combined with Erchen Decoction on Immune Function and Inflammatory Factor Levels in Lung Cancer Patients after Radical Operation

目的:观察腕踝针联合二陈汤对肺癌根治术后患者免疫功能、炎症因子水平的影响。方法:80例肺癌根治术后患者按照随机数字表法分为对照组和观察组各40例。对照组给予常规治疗方案,包括口服复方甲氧那明胶囊及开展综合呼吸训练。观察组在对照组治疗基础上给予腕踝针联合二陈汤口服。治疗后比较两组患者咳嗽喘息中医证候积分、疼痛及行为状态评分、免疫因子水平、炎症因子水平、肺功能水平、术后康复时间、运动耐力、生活质量评分、炎症因子水平。结果:治疗后,两组患者咳嗽喘息中医证候积分及疼痛评分均显著降低,行为状态评分显著升高;观察组中医证候积分及疼痛评分均显著低于同期对照组,行为状态评分高于同期对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者免疫因子即CD3^(+)、CD4^(+)、CD4^(+)/CD8均显著升高,且观察组以上指标均高于同期对照组(P<0.05)。治疗后,两组患者炎症因子即白细胞介素-6、肿瘤坏死因子-α、C反应蛋白水平均显著降低,且观察组以上指标显著低于同期对照组(P<0.05)。治疗后,两组患者肺功能指标即第1秒用力呼气容积、最大呼气峰流速均显著升高,且观察组高于同期对照组(P<0.05)。观察组术后拔除导管时间、住院时间、6分钟步行距离均少于对照组(P<0.05)。治疗后,两组患者生活质量各维度即角色功能、认知功能、情绪功能、社会功能评分均显著升高,且观察组显著高于同期对照组(P<0.05)。结论:腕踝针联合二陈汤可显著改善肺癌根治术后患者症状及行为状态,恢复免疫功能,减轻炎症反应,提高肺功能,加快康复进程和增强运动耐力,并可提高生活质量,且安全性高。

Objective:To observe the effect of carpus-ankle acupuncture combined with Erchen Decoction on the immune function and inflammatory factor levels of patients after radical operation of lung cancer.Methods:80 patients with lung cancer after radical operation were divided into control group and observation group with 40 cases in each group according to the random number table method.The control group was given routine treatment,including oral administration of compound methoxyphenamine capsules and comprehensive breathing training.On the basis of the control group treatment,the observation group was given carpus-ankle acupuncture and Erchen Decoction orally.After treatment,cough and wheezing TCM syndrome scores,pain and behavioral status scores,immune factor levels,inflammatory factor levels,lung function levels,postoperative recovery time,exercise tolerance,quality of life scores,and inflammatory factor levels were compared between the two groups.Results:After treatment,the cough and wheezing TCM syndrome scores and pain scores of the two groups were significantly reduced,and the behavioral status scores were significantly increased.The TCM syndrome scores and pain scores in the observation group were significantly lower than those in the control group,and the behavioral status scores in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).After treatment,the immune factors CD3+,CD4+,and CD4+/CD8 in the two groups were significantly increased,and the above indicators in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of inflammatory factors in the two groups,including interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α),and C-reactive protein(CRP)levels were significantly reduced,and the above indicators in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the pulmonary function indexes of the two groups,including the forced expiratory volume in one second(FEV1)and peak expiratory flow(PEF),were significantly increased,and the observation group was higher than those of the control group in the same period(P<0.05).The catheter removal time,hospitalization time,and 6-minute walking distance in the observation group were all shorter than those in the control group(P<0.05).After treatment,the scores of all dimensions of quality of life in the two groups,which include role function,cognitive function,emotional function,and social function,were significantly increased,and the observation group was significantly higher than that of the control group(P<0.05).Conclusion:Carpus-ankle acupuncture combined with Erchen decoction can significantly improve the symptoms and behavior status of patients after radical operation for lung cancer,restore immune function,reduce inflammation,improve lung function,speed up the recovery process and enhance exercise endurance,and can improve the quality of life,with good reliability.
