右美托咪定联合罗哌卡因腹横肌平面阻滞在腹腔镜结直肠癌根治术中的应用效果

Application Effect of Dexmedetomidine Compounded with Ropivacaine Transversus Abdominis Plane Block in Laparoscopic Radical Colorectal Cancer Surgery

目的 探讨右美托咪定联合罗哌卡因腹横肌平面阻滞(TAPB)在腹腔镜结直肠癌根治术中的应用效果。方法 选取2020年6月至2022年9月南阳市第二人民医院收治的96例行腹腔镜结直肠癌根治术患者,随机分为对照组和观察组,各48例。对照组接受罗哌卡因TAPB;观察组接受右美托咪定联合罗哌卡因TAPB。比较两组患者镇痛效果、镇静效果、恢复情况、疼痛程度、术后恢复质量以及不良反应发生情况。结果 两组镇痛效果相比,观察组明显较优(P<0.05);两组患者恢复情况相比,观察组明显较优(P<0.05);术后12、24 h两组患者疼痛程度相比,观察组均较低,且观察组镇静优于对照组(P<0.05);术后3 d,两组恢复质量及睡眠质量均改善,观察组优于对照组(P<0.05);两组患者不良反应发生率相比无统计学差异(P>0.05)。结论 右美托咪定联合罗哌卡因TAPB应用于腹腔镜结直肠癌根治术,可促进患者术后生理功能恢复,提高睡眠质量,缩短住院时间,减轻患者疼痛感。

Objective To investigate the application effect analysis of dexmedetomidine compounded with ropivacaine transversus abdominis plane block(TAPB)in laparoscopic radical colorectal cancer surgery.Methods Ninety-six patients admitted to Nanyang Second People’s Hospital for laparoscopic radical colorectal cancer from June 2020 to September 2022 were selected and divided into control group and observation group according to the random number table method,with 48 cases in each group.The control group received ropivacaine for TAPB,the observation group received dexmedetomidine compounded with ropivacaine for TAPB.The analgesic effect,sedation effect,recovery,pain level,quality of postoperative recovery and the occurrence of adverse reactions were compared between the two groups.Results The analgesic effect of the observation group was better than that of the control group(P<0.05).The recovery of the observation group was better than that of the control group(P<0.05).At 12 and 24 hours after operation,the degree of pain in the observation group were lower than those in the control group,and the degree of sedation were better than the control group(P<0.05).Three days after operation,the recovery quality and sleep quality improved in both groups,and the observation group were better than the control group(P<0.05).Compared with the incidence of adverse reactions in both groups,the difference was not statistically significant(P>0.05).Conclusion Dexmedetomidine compounded with ropivacaine in TAPB applied to laparoscopic radical colorectal cancer can improve the postoperative analgesic effect of patients,promote the recovery of patients’postoperative physiological functions,improve sleep quality and shorten the hospital stay.
