12例全膝关节置换术患者疼痛灾难化真实体验的质性研究

Qualitative research on the real experience of pain disaster in 12 patients undergoing total knee replacement

目的 探讨全膝关节置换患者的疼痛感受及内心体验,明确患者疼痛管理需求,提高患者疼痛管理水平。方法 采用目的抽样法,于2022年9—11月在临沂市人民医院选取创伤二科就诊的12例全膝关节置换患者进行半结构式访谈,采用Colaizzi七步分析法分析资料并提取主题。结果 共提炼5个主题:疼痛体验深刻、日常生活受限、自我应对与管理无效、自我价值感丧失、支持系统薄弱。结论 全膝关节置换患者在疼痛体验方面存在诸多困扰与负担,医护人员应针对其疼痛灾难化体验,转变其疼痛灾难化认知,提高疼痛管理水平的同时协助患者建立有效的社会支持系统,促进其身心康复。

Objective To investigate the pain feeling and inner experience of patients undergoing total knee replacement,so as to clarify the pain management needs of patients and improve the pain management level of patients.Methods With the method of objective sampling,12 patients undergoing total knee replacement in the DepartmentⅡof Traumatology in Linyi People’s Hospital from September to November 2022 were selected for semi-structured interviews.By Colaizzi seven-step analysis method,the data were analyzed and the themes were extracted.Results A total of 5 themes were extracted,namely profound pain experience,limited daily life,ineffective self-coping and management,loss of self-worth and weak support system.Conclusion There are many troubles and burdens in the aspect of pain experience in patients undergoing total knee replacement.Medical staff shall change their perception of pain disaster according to their pain disaster experience,improve the level of pain management,and help patients establish an effective social support system,so as to promote their physical and mental rehabilitation.
