摘要 目的针对当前儿童痉挛型脑瘫(Spastic Cerebral Palsy,SCP)患者对下肢运动功能康复的巨大需求,依据临床指导意见设计一款被动康复型的下肢康复训练设备。方法设备由控制系统、编程推杆和外骨骼支具等组成。在系统的控制下编程推杆带动外骨骼支具进行相应模式的被动康复运动。根据医疗器械设备的相关规定进行样机的设计、制作和测试,为验证设备的性能,对角度参数、角速度、臀宽和负载进行测量。结果设备的结构设计与功能参数均符合相关法律法规的规定,满足设计的预期目标。设备可使患儿髋臼骨与股骨头在50°~110°范围内,以5~16°/s进行髋关节康复运动,能有效帮助患儿进行髋关节的持续被动康复训练。结论本文研发的被动康复型的下肢康复训练设备可用于儿童SCP髋关节脱位术后的康复训练,干预髋关节发育不良进程,降低髋关节脱位风险,提升患儿的下肢运动能力,具有正确性和可行性。 Objective In response to the great demand of spastic cerebral palsy patients for lower limb motor function rehabilitation in children with spastic cerebral palsy(SCP),to design a passive rehabilitation type lower limb rehabilitation training device according to clinical guidance.Methods The device was composed of control system,coding actuator and exoskeleton support.Under the control of the system,the coding actuator drove the exoskeleton support to perform passive rehabilitation movements in the corresponding mode.The prototype was designed,manufactured and tested according to the relevant regulations of medical devices.In order to verify the performance of the equipment,the angular parameters,angular velocity,hip width and load were measured.Results The structural design and functional parameters of the equipment were in line with the provisions of relevant laws and regulations,and met the expected objectives of the design.The equipment could make the acetabular bone and femoral head in the range of 50°to 110°,at the speed from 5°/s to 16°/s for hip rehabilitation exercise,which could effectively help children to carry out continuous passive rehabilitation training of hip joint.Conclusion The passive rehabilitation lower limb rehabilitation training equipment developed in this paper can be used for rehabilitation training after SCP hip dislocation in children,to intervene in the process of hip dysplasia,reduce the risk of hip dislocation prevention,and improve the children’s lower limb movement ability,which is correct and feasible.
