人血管生成素样蛋白4在肝癌组织中的表达及其临床价值

Expression of human angiopoietin-like protein 4 in liver cancer tissues and its clinical value

目的研究人血管生成素样蛋白4(ANGPTL4)在肝癌组织中的表达其临床价值。方法选取72例肝癌患者为研究对象,采用免疫组织化学法检测肝癌组织和癌旁组织的ANGPTL4表达情况,分析ANGPTL4表达与肝癌临床病理特征的关系,随访2年,记录不同ANGPTL4表达的肝癌患者的复发情况,采用Kaplan-Meier法分析肝癌患者2年生存率。结果ANGPTL4在肝癌组织的阳性率为81.94%,明显高于癌旁组织的16.67%(χ^(2)=61.37,P<0.05);不同肿瘤大小、CNLC分期、淋巴结转移、血管侵犯和门静脉侵袭的肝癌患者的ANGPTL4阳性率比较,差异均有统计学意义(χ^(2)分别=5.55、4.48、4.95、5.11、6.50,P均<0.05);随访2年,肝癌患者复发23例,复发率为31.94%;复发的肝癌患者ANGPTL4阳性率为95.65%,明显高于未复发患者的75.51%(χ^(2)=4.29,P<0.05);ANGPTL4阴性表达者的2年生存率和中位生存时间分别为38.50%和21.30个月,ANGPTL4阳性表达者的2年生存率和中位生存时间为17.78%和16.90个月,两组2年生存率比较,差异有统计学意义(χ^(2)=6.20,P<0.05)。结论肝癌患者ANGPTL4阳性率明显高于癌旁组织,其阳性率与肝癌患者多种临床病理特征有关,且不同ANGPTL4表达的肝癌患者复发率、生存率存在差异,有望成为临床肝癌预后评估的有效指标。

Objective To study the expression of human angiopoietin-like protein 4(ANGPTL4)in liver cancer tis⁃sues and its clinical value.Methods A total of 72 patients with liver cancer were enrolled as the research objects.The expression of ANGPTL4 in liver cancer tissues and para-cancerous tissues was detected by immunohistochemistry.The re⁃lationship between ANGPTL4 and clinicopathological characteristics of liver cancer was analyzed.All were followed up for 2 years to record recurrence in liver cancer patients with different ANGPTL4 expression.The 2-year survival rate of liver cancer patients was analyzed by Kaplan-Meier.Results The positive rate of ANGPTL4 in liver cancer tissues was 81.94%,which was significantly higher than that in para-cancerous tissues(16.67%)(χ^(2)=61.37,P<0.05).The differenc⁃es in positive rate of ANGPTL4 among liver cancer patients with different tumor sizes,CNLC staging,lymph node metasta⁃sis,vascular invasion and portal vein invasion were statistically significant(χ^(2)=5.55,4.48,4.95,5.11,6.50,P<0.05).With⁃in 2 years of follow-up,there were 23 cases with recurrence,with recurrence rate of 31.94%.The positive rate of ANG⁃PTL4 in patients with recurrence was significantly higher than that without recurrence(95.65%vs 75.51%)(χ^(2)=4.29,P<0.05).The 2-year survival rate and median survival time in liver cancer patients with negative-expression of ANGPTL4 were 38.50%and 21.30 months,while 17.78%and 16.90 months in liver cancer patients with positive-expression of ANG⁃PTL4.The 2-year survival rate between two groups was significantly different(χ^(2)=6.20,P<0.05).Conclusion The posi⁃tive rate of ANGPTL4 in liver cancer tissues is significantly higher than that in para-cancerous tissues.The positive rate is related to clinicopathological characteristics of liver cancer patients.There are differences in the recurrence rate and survival rate among liver cancer patients with different ANGPTL4 expression,which can be to an effective evaluation in⁃dex for clinical prognosis of liver cancer.
