围手术期胃癌患者症状群与生活质量的相关性研究

Correlation between symptom clusters and quality of life in patients with gastric cancer during perioperation

目的:调查围手术期胃癌患者3个时间点的症状群及生活质量的纵向变化情况,并分析二者的相关性。方法:选取2021年10月至2022年6月在山东省济南市某三级甲等医院胃肠外科拟行手术治疗的198例胃癌患者作为调查对象。分别在患者入院后第1天、术后2~4天和术后1个月,采用一般资料调查表、安德森症状评估量表胃肠道肿瘤特异性模块、癌症治疗功能评价普适性量表收集资料并分析数据。结果:围手术期胃癌患者共存在消化道症状群、能量缺乏症状群、心理症状群及手术相关症状群4个症状群;3个时间点的症状群严重程度得分(除手术相关症状群外)、生活质量各维度得分及总得分比较,差异有统计学意义(P均<0.05);各症状群严重程度得分(除手术相关症状群外)均与生活质量总得分呈负相关。结论:围手术期胃癌患者存在多个症状群,各症状群既保持相对稳定又存在动态变化;症状群能够影响患者的生活质量。研究提示,通过对症状群的管理,有望提升围手术期胃癌患者的生活质量。

Objective:To investigate the Longitudinal changes about symptom clusters and quality of life in perioperative patients with gastric cancer at three-time points,and to explore the correlations between them.Methods:From October 2021 to June 2022,a total of 198 patients who planned to undergo surgical treatment were selected in the gastrointestinal surgery ward of a tertiary grade A hospital in Jinan,Shandong province.On the 1st day after admission(T1),2-4 days after surgery(T2)and 1 month after surgery(T3),the general information questionnaire,the M.D.Anderson Symptom Inventory-Gastrointestinal Cancer Module,the Functional Assessment of Cancer Therapy-General were used to collect the relevant data.Results:There were 4 symptom clusters in perioperative gastric cancer patients,namely,the digestive tract symptom cluster,the energy deficiency symptom cluster,the psychological symptom cluster,and the surgeryrelated symptom cluster.There were significant differences in the severity scores of symptom clusters(except surgery-related symptom cluster)and in the total scores of quality of life at three-time points in the perioperative period(all P<0.05).The severity score of each symptom cluster(except surgery-related symptom cluster)was negatively correlated with the total score of quality of life.Conclusion:There are many symptom clusters in perioperative gastric cancer patients,and each symptom cluster remains relatively stable and changes dynamically,which can affect the quality of life of the patients.This study suggests that the management of symptom clusters is expected to improve the quality of life of patients with perioperative gastric cancer.
