县域医共体背景下远程医疗推广策略研究

Evolutionary Game Study on the Promotion Strategy of Telemedicine in the Context of County Medical Community

远程医疗既能有效促进优质医疗资源下沉,又能降低医疗成本。远程医疗的推广,对于提高国家整体医疗水平具有重要促进作用,有利于提升人民群众的幸福感与获得感。本文在县域医疗共同体远程医疗建设背景下,基于供需视角构建县域医共体与患者之间以及医共体内部的演化博弈模型,探讨影响远程医疗推广因素。研究发现:基层医院误诊率会影响患者基层就诊的意愿;医保报销比例会影响县域医共体推广远程医疗的意愿;县域医共体内部合理的收益分配比例可以有效增强核心医院与基层医院间的合作意愿。基于此,应降低误诊率、完善医保报销比例、合理分配医共体内部收益,以推进远程医疗服务,从而落实分级诊疗制度。

Telemedicine can effectively promote the sinking of high-quality medical resources while also reducing medical costs.The promotion of telemedicine plays an important role in improving the overall medical level of the country,and is conducive to enhancing the happiness and sense of gain of the people.Under the background of the telemedicine construction of the county-level medical community,this paper constructs an evolutionary game model between the county-level medical community and patients as well as within the medical community based on the perspective of supply and demand,and explores the factors that affect the promotion of telemedicine.Research has found that the misdiagnosis rate in grassroots hospitals can affect patients'willingness to seek medical treatment at the grassroots level;The proportion of medical insurance reimbursement will affect the willingness of county-level medical communities to promote telemedicine;A reasonable proportion of income distribution within the county medical community can effectively enhance the willingness to cooperate between core hospitals and grassroots hospitals.Based on this,it is necessary to reduce the misdiagnosis rate,improve the proportion of medical insurance reimbursement,and reasonably allocate internal benefits within the medical community to promote remote medical services,thus implementing a hierarchical diagnosis and treatment system.
