妊娠期高血压发生的危险因素分析及饮食与药物干预效果 被引量:1

摘要 目的探讨妊娠期高血压发生的危险因素分析及饮食与药物干预效果。方法选择2019年3月至2021年9月于我院进行治疗的妊娠期高血压产妇230例作为观察组,选择同期产检正常产妇220例作为对照组。通过自编问卷及临床资料对妊娠期高血压发生影响因素进行单因素、多因素Logistic回归分析。对照组进行常规干预,观察组进行饮食及药物干预,比较干预后两组血压及Hcy水平。结果单因素分析结果表明:妊娠期高血压发生影响的因素与年龄、孕前体质量指数、既往高血压病史、产前定期检查、文化水平、不良心理情绪、不良生活方式差异有统计学意义(P<0.05);多因素Logistic回归分析结果表明:年龄(β=2.933,OR=3.738,95%CI=0.283~4.391)、孕前体质量指数(β=1.214,95%CI=2.481~6.314)、既往高血压病史(β=1.561,95%CI=5.682~8.452)、产前定期检查(β=1.452,95%CI=3.235~4.573)以及不良心理情绪(β=1.561,95%CI=5.682~8.453)是影响妊娠期高血压发生的危险因素(P<0.05);干预后观察组的DBP、SBP及Hcy水平均低于对照组(P<0.05)。结论妊娠期高血压发生的危险因素较多,根据不同患者的特点和危险因素进行针对性干预,加强饮食与药物干预可有效控制产妇血压及Hcy水平。 Objective To explore the risk factors of pregnancy-induced hypertension and the effect of diet and drug intervention.Methods A total of 230 pregnant women with hypertensive disorder complicating pregnancy treated in Hospital from March 2019 to September 2021 were selected as the observation group,and 220 normal women were selected as the control group.The influencing factors of pregnancy induced hypertension were analyzed by univariate and multivariate Logistic regression through self-designed questionnaires and clinical data of the two groups.The observation group was given routine intervention,and the observation group was given diet and drug intervention.The blood pressure and Hcy levels of the two groups were compared after intervention.Results The univariate results showed that the influencing factors of pregnancy-induced hypertension were statistically significant with age,pre-pregnancy body mass index,previous history of hypertension,regular prenatal check-up,education level,bad psychological mood and bad lifestyle(P<0.05).Multivariate Logistic regression analysis showed that:age(β=2.933,OR=3.738,95%CI=0.283-4.391),pre-pregnancy body mass index(β=1.214,95%CI=2.481-6.314),previous hypertension history(β=1.561,95%CI=5.682-8.452),regular prenatal checkups(β=1.452,95%CI=3.235-4.573)and bad psychological mood(β=1.561,95%CI=5.682-8.453)are the risk factors of pregnancy-induced hypertension(P<0.05).After intervention,the levels of DBP,SBP and Hcy in the observation group were lower than those in the control group(P<0.05).Conclusion There are many risk factors of pregnancy-induced hypertension.According to the characteristics and risk factors of different patients,targeted intervention and strengthening diet and drug intervention can effectively control maternal blood pressure and Hcy level.
