血清NT-proBNP、H-FABP水平联合检测在急性心力衰竭患者诊断中的应用价值

Application value of combined detection of serum NT-proBNP and H-FABP levels in diagnosis of patients with acute heart failure

目的探讨血清心型脂肪酸结合蛋白(H-FABP)、前体脑钠肽(NT-proBNP)水平联合检测在急性心力衰竭患者诊断中的应用价值.方法选取江苏省第二中医院2019年4月-2021年2月收治的急性心力衰竭患者102例设为研究组,均经临床检查确诊,另选同期具有胸痛症状且最终诊断为非急性心力衰竭者104例设为对照组,均行血清H-FABP、NT-proBNP表达水平检测,对比两组血清H-FABP、NT-proBNP表达水平,比较单一检测及联合检测在急性心力衰竭患者中的诊断价值.结果研究组血清H-FABP、NT-proBNP表达水平较对照组高(P<0.05);经血清H-FABP水平检测确诊急性心力衰竭患者86例,经血清NT-proBNP水平检测确诊急性心力衰竭患者87例,经二者联合检测确诊急性心力衰竭患者99例;在急性心力衰竭的诊断中,血清H-FABP、NT-proBNP水平联合检测的诊断特异度为97.12%、灵敏度97.06%、准确度97.09%,均较单一检测高(P<0.05),且联合检测误诊率2.88%、漏诊率2.94%,均较单一检测低(P<0.05).结论血清H-FABP、NT-proBNP水平联合检测,可显著提高急性心力衰竭诊断特异度、灵敏度及准确度,降低误诊率及漏诊率,有助于急性心力衰竭早期诊断.

Objective To explore the value of combined detection of serum heart-type fatty acid binding protein(H-FABP)and precursor brain natriuretic peptide(NT-proBNP)levels in the diagnosis of patients with acute heart failure.Methods A total of 102 patients with acute heart failure from April 2019 to February 0221 were enrolled in the study group.All of them were confirmde by clinical diagnosis.Another 104 patients with chest pain and the final diagnosis of non-acute heart failure were selected as controlsT.he levels of serum H-FABP and NT-proBNP were detected in the group.The expression levels of serum H-FABP and NT-proBNP were compared between the two groups.The diagnostic value of single and comibned detection in patients with acute heart failure was comparde.Results The expression levels of serum H-FABP and NT-proBNP in the study group were higher than those in the control groupP(<0.05).86 patients with acute heart failure were diagnosed by serum H-FABP level.99 patients with acute heart failure were diagnosed by combined detection.In the diagnosis of acute heart failure,the diagnostic specificity of combined detection of serum H-FABP and NT-proBNP levels was 97.12%,sensitivity 97.06%,and accuracy 97.09%,both were higher than single detection(P<0.05),and the combined detection misdiagnosis rate was 2.88%,and the missed diagnosi srate was 2.94%was lower than single detectionP(<0.05).Conclusion Combined detection of serum H-FABP and NT-proBNP levels can significantly improve the diagnostic specificity,sensitivity anda ccuracy of acute heart failure,reduce the rate of misdiagnosis and missed diagnosis,and contribute to the early diagnosis of acute heart failure.
