穴位推拿结合理疗在腰椎间盘突出症坐骨神经痛的护理效果

Nursing effect of acupoint massage combined with physical therapy on lumbar disc herniation and sciatica

目的探讨腰椎间盘突出症坐骨神经痛联合应用穴位推拿与理疗的护理效果.方法选择2020年8月-2021年8月汕头市潮阳区大峰医院收治的186例腰椎间盘突出症坐骨神经痛患者,根据电脑随机盲选法,分为观察组和对照组,每组各93例.对照组应用常规护理,在此基础上,观察组结合穴位推拿与理疗.干预前、干预后3个月,比较两组五点口述分级评分法(VRS-5)评分、简易麦吉尔疼痛量表(PRI)评分、外周血活性氧(ROS)、神经生长因子(NGF)、单核细胞趋化蛋白-1(MCP-1)浓度、白介素-6(IL-6)、干扰素-γ(IFN-γ)、超敏C反应蛋白(hs-CRP)、Oswestry功能障碍指数问卷表(ODI)评分、日本骨科协会评估治疗分数(JOA)、直腿抬高度;比较干预效果.结果干预后3个月,两组VRS-5评分、PRI评分、ROS、MCP-1、IL-6、IFN-γ、hs-CRP、ODI评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组NGF、JOA评分、直腿抬高度均升高,且观察组高于对照组,差异有统计学意义(P<0.05);观察组干预优良率(81.72%)高于对照组(65.59%),差异有统计学意义(P<0.05).结论腰椎间盘突出症坐骨神经痛联合应用穴位推拿与理疗护理,效果理想,可显著减轻疼痛症状,降低腰椎炎症反应及氧化应激反应,改善腰椎功能,促进康复.

Objective To explore the nursing effect of combined acupoint massage andp hysical therapy for lumbar disc herniation and sciatica.Methods A total of 186 patients with lumbar disc herniation and sciatica who were admitted to our hospital from August 2020 to August 2021 were selected and divided into observation group and control group,with 93 cases in each group,according to the computerized random blind selection method.The control group received routnie nursing care.On this basis,the observation group combineda cupoint massage and physical therapy.Before the intervention and 3 months after the intervention,compare the two groups of five-point oral grading scoring method(VRS-5)score,simple McGill pain scale P(RI)score,peripheral blood reactive oxygen species(ROS),nerve growth factor(NGF),Monocyte chemoattractant protein-1(MCP-1)concentration,interleukin-6(IL-6),interferon-γ(IFN-γ),high-sensitivity C-reactive protein(hs-CRP),Oswestry dysfunction Index questionnaire(ODI)score,Japanese Orthopedic Association Assessment Treatment Score(JOA),straight leg lift height;compare intervention effects.Results 3 months after the intervention,the scores of VRS-5,PRI,ROS,MCP-1,IL-6,IFN-γ,hs-CRP,and ODI in the two groups decreased,and the observation group was lower than the control group.There was statistical significance P(<0.05).The NGF,JOA score,and the height of straight leg lif tincreased in the two groups,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The observation group's intervention excellent rate(81.72%)was higher than the control group(65.59%),the difference was statistically significant(P<0.05).Conclusion The combined application of acupoint massage and physical thearpy for lumbar disc herniation and sciatica has ideal results.It can significantly reduce pain symptoms,reduce lumbar inflammation and oxidative stress,improve lumbar spine function,and promote recovery.
