循证理念指导下无缝护理模式在呼吸衰竭患者呼吸道管理中的应用价值

Application value of seamless nursing model under the guidance of evidence-based concept on respiratory tract management of patients with respiratory failure

目的探讨循证理念指导下无缝护理模式在呼吸衰竭患者呼吸道管理中的应用价值。方法选取2020年1月至2021年12月江西省上饶市人民医院收治的122例呼吸衰竭患者为研究对象,应用随机数字表法分为观察组(n=61)和对照组(n=61),对照组给予常规护理,观察组在对照组基础上给予循证理念指导下无缝护理。比较两组患者干预前后的血气指标、生存质量[圣乔治呼吸问卷(SGRQ)]的变化和不良事件(管道脱落、气囊漏气、气道分泌物堵塞)及呼吸机相关性肺炎(VAP)的发生情况。结果观察组护理后动脉氧分压高于对照组,差异有统计学意义(P<0.05);观察组护理后肺泡-动脉血氧分压、SGRQ各项评分低于对照组,差异有统计学意义(P<0.05);观察组护理期间管道脱落、气囊漏气、气道分泌物堵塞及VAP发生率低于对照组,差异有统计学意义(P<0.05)。结论循证理念指导下无缝护理模式能改善呼吸衰竭患者血气分析指标、减少不良事件及VAP发生风险,有利于提高其生存质量。

Objective To evaluate the application value of seamless nursing model under the guidance of evidence-based concept on respiratory tract management of patients with respiratory failure.Methods A total of 122 patients with respiratory failure admitted to Shangrao People's Hospital,Jiangxi Province from January 2020 to December 2021 were selected as the study objects and they were divided into observation group(n=61)and control group(n=61)by the random number table method.The control group was given routine nursing,while the observation group was given seamless nursing under the guidance of evidence-based concept on the basis of the control group.Blood gas indexes,quality of life(St George's respiratory questionnaire[SGRQ])changes,adverse events(tube loss,air sac leakage,airway secretion blockage)and ventilators associated pneumonia(VAP)were compared between the two groups before and after intervention.Results The arterial oxygen partial pressure in the observation group was higher than that in the control group after nursing,and the difference was statistically significant(P<0.05).Alveolar-arterial oxygen partial pressure and SGRQ scores in the observation group were lower than those in the control group after nursing,and the differences were statistically significant(P<0.05).The incidence of tube shedding,air bag leakage,airway secretion blockage and VAP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Seamless nursing model under the guidance of evidence-based concept can improve blood gas analysis indicators,reduce the adverse events and VAP,and help enhance the quality of life of patients with respiratory failure.
