基于肾功能、心功能指标评价温阳利水汤治疗慢性心肾综合征患者的效果

The Effect of Wenyang Lishui Decoction on Patients with Chronic Cardiorenal Syndrome Based on Renal Function and Heart Function Indexes

目的:探讨基于肾功能、心功能指标评价温阳利水汤治疗慢性心肾综合征患者的效果。方法:选取2019年10月-2022年8月九江市中医医院收治的慢性心肾综合征患者92例,以随机数字表法分组,其中46例为对照组(采用常规治疗),46例为观察组(采用常规治疗联合温阳利水汤治疗),两组均持续治疗1个月。比较两组各项临床指标。结果:治疗后,观察组临床总有效率高于对照组(P<0.05);与治疗前比较,治疗后两组中医症候积分(尿少、气短、畏寒、胸闷、疲倦乏力、浮肿)、血肌酐(Scr)、血尿素氮(BUN)、胱抑素C水平、左心室内径、氨基末端脑钠肽前体(NT-proBNP)均降低,观察组均更低(P<0.001);两组肌酐清除率(Ccr)水平、左室射血分数(LVEF)均升高,观察组均更高(P<0.001)。治疗期间,观察组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论:温阳利水汤有助于增强慢性心肾综合征患者临床治疗效果,缓解临床症状,减少不良反应的发生,这可能与改善患者的肾功能、心功能等因素有关。

Objective:To explore the evaluation of the effect of Wenyang Lishui Decoction on patients with chronic cardiorenal syndrome based on renal function and heart function indexes.Method:Ninety-two patients with chronic cardiorenal syndrome admitted to Jiujiang Hospital of Traditional Chinese Medicine from October 2019 to August 2022 were selected,the samples(92 patients)were grouped according to the random number table method,of which 46 cases for the control group(receiving conventional treatment),46 cases for the observation group(receiving conventional treatment combined with Wenyang Lishui Decoction),the treatment lasted for one month in both groups.The various clinical indicators were compared between the two groups.Result:After treatment,the total clinical response rate in the observation group was higher than that in the control group(P<0.001);compared with before treatment,TCM symptom scores(oliguria,shortness of breath,chills,chest tightness,fatigue,edema),serum creatinine(Scr),blood urea nitrogen(BUN),cystatin C level,left ventricular internal diameter,N-terminal precursor brain natriuretic peptide(NT-proBNP)of the two groups after treatment were decreased,those of the observation group were lower(P<0.001);the levels of creatinine clearance rate(Ccr)and left ventricular ejection fraction(LVEF)were increased in both groups,those of the observation group were higher(P<0.001).During the treatment period,compared with the overall incidence of adverse effects in the control group,the observation group was even lower,the difference was statistically significant(P<0.05).Conclusion:Wenyang Lishui Decoction in the treatment of patients with chronic cardiorenal syndrome is helpful to enhance the clinical treatment effect,relieve the clinical symptoms,and reduce the occurrence of adverse reactions,which may be related to improve the renal function and heart function of the patients.
