高龄孕妇建立全程跟踪护理管理改善产后盆底功能的实践

Practice of establishing whole-course follow-up nursing management to improve postpartum pelvic floor function in elderly pregnant women

目的分析在高龄孕妇护理中,全程跟踪护理管理的具体方法,并对其改善产后盆底功能的实践进行研究.方法此研究为类实验研究,按照纳入标准与排除标准选择郴州市第四人民医院产科72例高龄孕妇.按照随机数字表法将选取的高龄孕妇分为对照组、研究组,每组36例,对照组按照常规护理方案开展护理干预,观察组采取全程跟踪护理管理方案进行护理干预,干预时间从入院前一直到出院后2个月,针对两组的评价指标包括POP-Q分度、护理满意度、母婴不良结局、盆底肌力、性生活质量.孕妇选取时间为2021年10月-2022年10月.结果对照组性生活质量各指标评分均低于研究组,数据分析有意义(P<0.05);与对照组的满意度对比显示,研究组较高(P<0.05);与对照组POP-Q分度相比,研究组优势显著,P<0.05,对比有意义;研究组盆底肌力优于对照组,差异分析有意义(P<0.05);对照组母婴不良结局发生率高于研究组,对比分析有意义(P<0.05).结论全程跟踪护理管理实施价值高,可以帮助患者提升性生活质量、护理满意度、盆底肌力,有效抑制母婴不良结局,可加以推广.

Objective To analyze the specific methods of the whole process tracking nursing management in the nursing of elderly pregnant women,and to study the practice of improving postpartum pelvic floor function.Methods This study was a quasi-experimental study.According to the inclusion and exclusion criteria,72 elderly pregnant women were selected from the obstetrics department of our hospital.According to the random number table method,the selected elderly pregnant women were divided into the control group and the study group,36 cases respectively.The control group carried out nursing intervention according to the routine nursing program,while the observation group carried out nursing intervention with the whole tracking nursing management program.The intervention time was from before admission to 2 months after postpartum discharge.The evaluation indexes for the two groups included POP-Q score,nursing satisfaction,maternal and infant adverse outcomes,pelvic floor muscle strength,and quality of sexual life.The selected time is from October 2021 to October 2022.Results The scores of all indicators of sexual life quality in the control group were higher than those in the study group,and the data analysis was significant(P<0.05).Compared with the control group,the satisfaction of the study group was higher(P<0.05);compared with the control group,the study group had a significant advantage in POP-Q indexing(P<0.05).The pelvic floor muscle strength of the study group was better than that of the control group,and the difference analysis was significant(P<0.05),.The incidence of adverse outcomes in the control group was higher than that in the study group,and the comparative analysis was significant(P<0.05).Conclusion The implementation value of whole-process tracking nursing management is high,which can help patients improve the quality of sexual life,nursing satisfaction,pelvic floor muscle strength,effectively inhibit the adverse outcomes of maternal and infant,and can be promoted.
