内镜下肠道植管中药灌肠结合美沙拉嗪对溃疡性结肠炎患者疗效及血清CRP水平的影响

Effect of traditional Chinese medicine enema combined with mesalazine on the efficacy and serum CRP level of patients with ulcerative colitis under endoscopy

目的探析内镜下肠道植管中药灌肠结合美沙拉嗪在溃疡性结肠炎患者中的应用效果.方法研究开展时间为2019年1月-2022年10月,选取淄博市博山区中医院脾胃科在此期间收治的85例溃疡性结肠炎患者为观察对象,均予以美沙拉嗪治疗,依据简单随机化法将其分为甲组和乙组,两组分别有患者42例、43例,前者予以中药传统灌肠法治疗,后者应用中药经内镜肠道植管灌肠法治疗,对比干预效果.结果相较于甲组(80.95%),乙组治疗总有效率(95.35%)更高(P<0.05);治疗前,甲组和乙组溃疡性结肠炎CRP、IL-6、TNF-α水平对比无明显差异(P>0.05);经过临床治疗后,两组炎症因子水平均有不同程度的下降,相较于甲组,乙组各项指标水平更低(P<0.05);治疗前,两组改良Mayo评分、IBDQ评分对比不具有差异性(P>0.05);在治疗后,相较于甲组,乙组改良Mayo评分(2.78±0.54)分更低,IBDQ评分(190.05±10.87)分更高(P<0.05).结论内镜下肠道植管中药灌肠结合美沙拉嗪治疗溃疡性结肠炎,临床疗效良好,可降低血清CRP水平,提高患者生活质量.

Objective To explore the effect of traditional Chinese medicine enema combined with mesalazine in patients with ulcerative colitis under endoscope.Methods From January 2019 to October 2022,85 patients with ulcerative colitis admitted by the Spleen and Gastroenterology Department of our hospital during this period were selected as the observation objects,all of whom were treated with mesalazine.According to the simple randomization method,they were divided into group A and group B,with 42 and 43 patients in each group,respectively.The former was treated with traditional Chinese medicine enema,while the latter was treated with traditional Chinese medicine enema through endoscopic intestinal tube planting,and the intervention effect was compared.Results Compared with group A(80.95%),the total effective rate of group B(95.35%)was higher(P<0.05);CRP,IL-6,TNF-αof ulcerative colitis in group A and group B before treatment,there was no significant difference in horizontal comparison(P>0.05);after clinical treatment,the levels of inflammatory factors in both groups decreased to varying degrees,and the levels of various indicators in group B were lower than those in group A(P<0.05);before treatment,there was no difference in the improved Mayo score and IBDQ score between the two groups(P>0.05);after treatment,compared with group A,the modified Mayo score(2.780.54)of group B was lower,and the IBDQ score(190.05±10.87)was higher(P<0.05).Conclusion The treatment of ulcerative colitis with traditional Chinese medicine enema and mesalazine under endoscope has a good clinical effect,which can reduce the level of serum CRP and improve the quality of life of patients.
