腹腔镜胆囊切除术治疗胆结石的临床效果分析

Clinical Analysis of Laparoscopic Cholecystectomy in the Treatment of Gallstones

目的探讨腹腔镜胆囊切除术治疗胆结石的临床效果。方法选取2019年1月—2021年12月山东省东营市第二人民医院收治的60例胆结石患者为研究对象,入院后以电脑随机法分为A(n=30)、B(n=30)两组,分别给予传统开腹手术及腹腔镜胆囊切除术治疗,对比两组手术治疗效果。结果B组手术时间、切口长度、术中出血量、腹腔引流量分别为(71.29±5.63)min、(1.61±0.27)cm、(16.71±2.68)mL、(27.94±5.57)mL均较A组的(86.55±5.89)min、(8.32±2.34)cm、(63.34±4.79)mL、(52.62±9.04)mL更优,差异有统计学意义(t=10.258、15.602、46.532、12.730,P<0.001)。与A组相比,B组首次下床时间、首次排气时间、伤口愈合时间、住院时间均更短,术后并发症总发生率、各时间节点疼痛评分均更低,差异有统计学意义(P<0.05)。结论腹腔镜胆囊切除术治疗胆结石兼具有效性及安全性,同时还可以显著缓解术后疼痛并加快康复速度。

Objective To explore the clinical effect of laparoscopic cholecystectomy in the treatment of gallstones.Methods 60 patients with gallstones admitted to the Second People's Hospital of Dongying City,Shandong Province from January 2019 to December 2021 were selected as the study subjects.After admission,they were randomly divided into two groups:A(n=30)and B(n=30)by computer.They were treated with traditional open surgery and laparoscopic cholecystectomy respectively,and the therapeutic effects of the two groups were compared.Results The operation time,incision length,intraoperative bleeding volume and peritoneal drainage volume of group B were(71.29±5.63)min,(1.61±0.27)cm,(16.71±2.68)mL and(27.94±5.57)mL respectively,which were better than those of group A(86.55±5.89)min,(8.32±2.34)cm,(63.34±4.79)mL and(52.62±9.04)mL,the difference was statistically significant(t=10.58,15.602,46.532,12.730,P<0.001).Compared with group A,group B had shorter time to get out of bed for the first time,exhaust time for the first time,wound healing time and hospitalization time,and lower total incidence of postoperative complications and pain scores at each time node,the difference was statistically significant(P<0.05)Conclusion Laparoscopic cholecystectomy for the treatment of gallstones is both effective and safe,and can significantly relieve postoperative pain and accelerate recovery.
