超声刀辅助下腮腺良性肿瘤包膜外切除的临床疗效分析

Clinical Analysis of Ultrasound-assisted Extracapsular Resection of Benign Parotid Tumors Disease

目的分析超声刀辅助下包膜外切除术对腮腺良性肿瘤的治疗效果。方法选取2017年1月—2021年12月江苏大学附属句容医院(句容市人民医院)口腔科收治的50例腮腺良性肿瘤患者,经随机数表法平均分组。对照组(n=25)经电刀行包膜外切除术治疗,观察组(n=25)于超声刀辅助下行包膜外切除术治疗。对比两组手术指标、面部神经功能和并发症发生情况。结果观察组手术时间、术后引流时间和住院时间短于对照组[(80.72±10.32)min vs(112.34±12.18)min、(44.16±7.18)h vs(60.72±12.24)h、(6.12±1.14)d vs(7.56±1.21)d],术中出血量和术后引流量少于对照组[(40.18±6.72)mL vs(54.72±8.16)mL、(64.24±5.44)mL vs(85.32±7.18)mL],并发症发生率低于对照组(4.00%vs 32.00%),差异有统计学意义(t=9.885、5.835、4.331、6.887、11.701,χ^(2)=4.878,P<0.05);两组面部神经功能对比,差异有统计学意义(Z=2.709,P<0.05)。结论予以腮腺良性肿瘤患者超声刀辅助下包膜外切除术,可改善手术指标和面部神经功能,防范并发症作用明显,有推广价值。

Objective To analyze the effect of ultrasound-assisted extracapsular resection on benign parotid tumors.Methods From January 2017 to December 2021,50 patients with benign tumor of parotid gland admitted to the Department of Stomatology of Jurong Hospital affiliated to Jiangsu University(Jurong People's Hospital)were randomly divided into two groups.The control group(n=25)was treated with extracapsular resection by electric knife,and the observation group(n=25)was treated with extracapsular resection assisted by ultrasonic knife.The operation indexes,facial nerve function and complications were compared between the two groups.Results The operation time,postoperative drainage time and hospital stay in the observation group were shorter than those in the control group[(80.72±10.32)min vs(112.34±12.18)min,(44.16±7.18)h vs(60.72±12.24)h,(6.12±1.14)d vs(7.56±1.21)d],and the intraoperative bleeding volume and postoperative drainage volume were lower than those in the control group[(40.18±6.72)mL vs(54.72±8.16)mL,(64.24±5.44)mL vs(85.32±7.18)mL],and the incidence of complications was lower than that of the control group[4.00%vs 32.00%],and the difference was statistically significant(t=9.885,5.835,4.331,6.887,11.701,χ^(2)=4.878,P<0.05).There was statistically significant difference in facial nerve function between the two groups(Z=2.709,P<0.05).Conclusion Ultrasound-assisted extracapsular resection for benign parotid tumors can improve surgical indicators and facial nerve function,and prevent complications significantly,which is of popularization value.
