加味真武汤治疗糖尿病肾病的疗效及对中医证候积分和肾功能的影响

The Efficacy of Jiaweizhenwu Decoction in the Treatment of Diabetic Nephropathy and the Effects on the Chinese Medicine Evidence Score and Renal Function

目的探究加味真武汤治疗糖尿病肾病的疗效及对中医证候积分和肾功能的改善作用。方法选择2019年12月—2021年12月南京市六合区中医院收治门诊及住院的糖尿病肾病患者60例作为研究对象,依据随机数表法将患者分为对照组(n=30,常规西药瑞格列奈治疗)与研究组(n=30,对照组用药基础上予加味真武汤治疗),比较两组临床疗效,并评估比较两组治疗前后中医证候积分,另检测患者治疗前后肌酐与尿蛋白定量,评估其肾功能。结果研究组临床有效率为93.33%,较对照组高,差异有统计学意义(P<0.05)。治疗后研究组患者乏力倦怠、畏寒肢冷、食少纳呆、肢体水肿、便溏泄泻与夜尿频多的中医证候积分分别为(0.89±0.31)分、(0.93±0.32)分、(0.90±0.35)分、(0.82±0.29)分、(0.86±0.30)分、(0.97±0.34)分,均较对照组治疗后的各项积分要低,差异有统计学意义(P<0.05)。治疗后,研究组患者肌酐与尿蛋白定量含量水平分别为(190.42±10.21)μmol/L、(1.19±0.32)g/24 h,均较对照组低,差异有统计学意义(P<0.05)。结论在糖尿病肾病的临床治疗中,采用加味真武汤治疗疗效可靠,能够显著改善患者临床症状与肾功能。

Objective To investigate the efficacy of Jiaweizhenwu decoction in the treatment of diabetic nephropathy and the improvement effect on the TCM evidence score and renal function.Methods Sixty patients with diabetic nephropathy who were admitted to the outpatient and inpatient clinics of Liuzhou District Hospital of Traditional Chinese Medicine from December 2019 to December 2021 were selected as a research object.Patients were divided into a control group(n=30,treated with conventional western drug Repaglinide)and a study group(n=30,treated with Jiaweizhenwu decoction on top of the control group)according to the random number table.The clinical efficacy of the two groups was observed and compared,and the post-treatment TCM points of the two groups were evaluated,and the creatinine and urine protein quantification of the patients before and after treatment were also measured to assess their renal function.Results The clinical effective rate of the study group was 93.33%,which was higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,the TCM syndrome scores of the study group included weakness and lassitude,coldness in the extremities,poor appetite,edema in the extremities,loose stools and diarrhea,and frequent nocturia were(0.89±0.31)points,(0.93±0.32)points,(0.90±0.35)points,(0.82±0.29)points,(0.86±0.30)points,and(0.97±0.34)points,which were lower than those of the control group,the difference was statistically significant(P<0.05).The levels of creatinine and urinary protein quantification in the study group were(190.42±10.21)μmol/L and(1.19±0.32)g/24 h,respectively,which were lower than those in the control group after treatment,the difference was statistically significant(P<0.05).Conclusion In the clinical treatment of diabetic nephropathy,the efficacy of treatment with Jiaweizhenwu decoction is reliable and can significantly improve the clinical symptoms and renal function of patients.
