尿蛋白和尿微量白蛋白在糖尿病肾病检验中的应用效果评价

Evaluation of the Effectiveness of Urine Protein and Urine Microalbumin in the Testing of Diabetic Nephropathy

目的研究尿蛋白和尿微量白蛋白在糖尿病肾病检验中的应用效果。方法将上海交通大学医学院附属瑞金医院卢湾分院2020年9月—2021年9月接收的100例糖尿病肾病患者纳入观察组,对照组为同期在本院进行体检的健康体检者100名。所有研究对象均进行尿蛋白和尿微量白蛋白检测并对比检测结果。结果观察组的尿蛋白(121.65±14.85)mg/L、尿微量白蛋白(44.05±5.89)mg/L均高于对照组,单一检测、联合检测的阳性检出率(26.00%、62.00%、82.00%)均高于对照组,差异有统计学意义(P<0.05);尿蛋白、尿微量白蛋白的阳性检出率随着病程的增加而升高,差异有统计学意义(P<0.05),联合检测的敏感度、准确率均为82.00%,漏诊率、阳性预测值、阴性预测值分别为18.00%、100.00%、0.00%。结论尿蛋白和尿微量白蛋白联合检测能够为临床诊治糖尿病肾病提供可靠参考数据。

Objective To study the effect of urine protein and urine microalbumin in the test of diabetic nephropathy.Methods 100 patients with diabetic nephropathy received by Luwan Branch of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from September 2020 to September 2021 were included in the observation group,and 100 healthy subjects who underwent physical examination in the hospital during the same period were included in the control group.All subjects were tested for urinary protein and urinary microalbumin and compared the test results.Results The urine protein(121.65±14.85)mg/L and urine microalbumin(44.05±5.89)mg/L in the observation group were higher than those in the control group,and the positive detection rates of single and combined tests(26.00%,62.00%,82.00%)were higher than those in the control group,the difference was statistically significant(P<0.05).The positive detection rate of urine protein and urine microalbumin increased with the increase of disease duration,the difference was statistically significant(P<0.05),and the sensitivity and accuracy rate of the combined test were 82.00%,and the leakage rate,positive predictive value,and negative predictive value were 18.00%,100.00%,and 0.00%,respectively.Conclusion The combined urine protein and urine microalbumin test can provide reliable reference data for clinical diagnosis and treatment of diabetic nephropathy.
