血清C肽、HbA1c联合检测在糖尿病患者诊断中的应用价值

Application Value of Serum C Peptide and HbA1c Combined Detection in the Diagnosis of Diabetic Patients

目的探讨血清C肽、糖化血红蛋白(HbA1c)联合检测在糖尿病患者诊断中的应用价值。方法选取2020年5月—2022年5月淄博市中医医院收治的101例糖尿病患者作为糖尿病组,另选择同期在本院体检的102名健康者作为健康体检组。所有研究对象均进行血清C肽、空腹血糖(FBG)、餐后2 h血糖(2 hPG)与HbA1c水平检查。比较糖尿病组、健康体检组的血清C肽、FBG、2 hPG与HbA1c水平,血清C肽、HbA1c单独及二者联合检测结果及诊断效能。结果与健康体检组比,糖尿病组血清FBG、2 hPG及HbA1c水平均较高,差异有统计学意义(P<0.05);糖尿病组血清C肽水平(0.56±0.11)μg/L低于健康体检组(1.77±0.24)μg/L,差异有统计学意义(t=46.099,P<0.05);经血清C肽、HbA1c单独检测的灵敏度(80.20%、79.21%)、准确率(79.31%、78.33%)、特异度(78.43%、77.45%)均低于二者联合检测(97.03%、97.54%、98.04%),差异均有统计学意义(χ^(2)=16.325、37.389、21.166,P<0.05)。而经血清C肽、HbA1c单独检测的灵敏度、准确率、特异度比较,差异无统计学意义(P>0.05)。结论血清C肽、HbA1c联合检测对糖尿病具有较高的灵敏度、准确率、特异度,整体诊断价值较高。

Objective To explore the application value of serum C peptide and glycosylated hemoglobin(HbA1c)combined detection in the diagnosis of diabetic patients.Methods A total of 101 diabetic patients admitted to Zibo Hospital of Traditional Chinese Medicine from May 2020 to May 2022 were selected as the diabetes group,and 102 healthy people who underwent physical examination in our hospital during the same period were selected as the health examination group.Serum C-peptide,fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG)and HbA1c levels were tested in all subjects.The levels of serum C-peptide,FBG,2 hPG and HbA1c were compared between the diabetes group and the healthy group.The detection results of serum C-peptide and HbA1c alone and combined with them were compared.Results The serum levels of FBG,2 hPG and HbA1c in diabetes group were higher than those in healthy group,and the difference was statistically significant(P<0.05).The serum C-peptide level in diabetic group(0.56±0.11)μg/L was lower than that in healthy group(1.77±0.24)μg/L,and the difference was statistically significant(t=46.099,P<0.05).The sensitivity(80.20%,79.21%),accuracy(79.31%,78.33%)and specificity(78.43%,77.45%)of serum C peptide and HbA1c alone were lower than those of serum C peptide and Hba1c combined(97.03%,97.54%,98.04%),the differences were statistically significant(χ^(2)=16.325,37.389,21.166,P<0.05).The sensitivity,accuracy and specificity of serum C peptide and HbA1c were not statistically significant(P>0.05).Conclusion The combined detection of serum C peptide and HbA1c has high sensitivity,accuracy,specificity and overall diagnostic value for diabetes.
