达格列净联合门冬胰岛素30在2型糖尿病患者中的应用研究

Study on the Application of Dapagliflozin Combined with Insulin Aspart 30 in Patients with Diabetes Mellitus Type 2

目的分析2型糖尿病(diabetes mellitus type 2,T2DM)患者应用达格列净联合门冬胰岛素30治疗取得的效果。方法选取2020年6月—2022年5月山东国欣颐养集团枣庄医院收治的T2DM患者102例,按照随机数表分为对照组(51例)与研究组(51例)。对照组应用门冬胰岛素30注射液皮下注射治疗,研究组在此基础上联合达格列净治疗。针对两组血糖、胰岛功能,以及不良反应进行对比。结果两组用药前空腹血糖(FPG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2 hPG)指标对比,差异无统计学意义(P>0.05);研究组用药8周时FPG、2 hPG、HbA1c指标均较对照组低,差异有统计学意义(P<0.05)。两组用药前空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞指数(HOMA-β)指标对比,差异无统计学意义(P>0.05);研究组用药8周时FINS、HOMA-β指标均较对照组高,HOMA-IR指标较对照组低,差异有统计学意义(P<0.05)。结论T2DM患者应用达格列净联合门冬胰岛素30治疗可以有效控制血糖水平,改善胰岛功能。

Objective To analyze the effect achieved by applying dapagliflozin combined with insulin aspart 30 in patients with diabetes mellitus type 2(T2DM).Methods One hundred and two patients with T2DM admitted to Zaozhuang Hospital of Shandong Guoxin Yiyang Group from June 2020 to May 2022 were selected and divided into a control group(fifty-one cases)and a study group(fifty-one cases)according to a random number table.The control group was treated with subcutaneous injection of insulin aspart 30,and the study group was treated with dapagliflozin on this basis.The two groups were compared for blood glucose,islet function,and adverse reactions.Results There was no difference between the two groups in terms of fasting plasma glucose(FPG),hemoglobin A1C(HbA1c)and 2 hours postprandial blood glucose(2 hPG)before medication(P>0.05).The FPG,2hPG and HbA1c indexes in the study group were lower than those in the control group at 8 weeks of medication,the difference was statistically significant(P<0.05).Fasting serum insulin(FINS),homeostasis model assessment-insulin resistance(HOMA-IR),homeostasis model assessmentβindex(HOMA-β)were compared before medication,and the difference was not statistically significant(P>0.05).At 8 weeks of medication,FINS and HOMA-βindexes in the study group were higher than those in the control group,and HOMA-IR indexes were lower,the difference was statistically significant(P<0.05).Conclusion The application of dapagliflozin combined with insulin aspart 30 in T2DM patients can effectively control blood glucose level and improve islet function.
