预见性护理对降低糖尿病肾病血液透析患者低血糖的效果研究

A Study on the Effectiveness of Anticipatory Care on Reducing Hypoglycemia in Hemodialysis Patients with Diabetic Nephropathy

目的研究糖尿病肾病血液透析患者接受预见性护理对预防低血糖的效果以及对患者血糖波动等相关指标的影响。方法选取2021年1月—2022年4月陆军厦门大学附属成功医院(陆军第七十三集团军医院)收治的84例糖尿病肾病患者作为研究对象,根据随机数表法将其分为两组,各42例。对照组实施常规护理,观察组实施预见性护理,比较两组的低血糖发生率、护理后血糖值、依从性以及患者满意度。结果与对照组相比,观察组的低血糖发生率更低,差异有统计学意义(P<0.05);观察组护理后的空腹血糖、餐后2 h血糖均低于对照组,差异有统计学意义(P<0.05);观察组患者在遵医嘱用药、合理运动、科学饮食、自我监测血糖方面的依从性评分均显著高于对照组,差异有统计学意义(P<0.05);观察组患者的满意度较对照组高,差异有统计学意义(P<0.05)。结论通过对糖尿病肾病血液透析患者实施预见性护理更有利于降低低血糖的发生风险,降低透析期间的血糖值,且有利于提高依从性和患者满意度。

Objective To study the effect of receiving anticipatory care on the prevention of hypoglycemia and the effect on patients′blood glucose fluctuation and other related indexes in hemodialysis patients with diabetic nephropathy.Methods From January 2021 to April 2022,84 patients with diabetes nephropathy who were admitted to the Success Hospital Affiliated to Xiamen University of the Army(the 73rd Army Group Hospital)were selected as the research objects,and they were divided into two groups according to the method of random number table,42 patients in each group.The control group was given routine nursing care,while the observation group was given predictive nursing care.The incidence of hypoglycemia,post nursing blood glucose values,compliance,and patient satisfaction were compared between the two groups.Results Compared with the control group,the incidence of hypoglycemia in the observation group was lower,with a statistically significant difference(P<0.05).The fasting blood glucose and 2 h postprandial blood glucose in the observation group after nursing were lower than those in the control group,with a statistically significant difference(P<0.05).The compliance scores of patients in the observation group in terms of medication compliance,reasonable exercise,scientific diet,and self monitoring blood glucose were significantly higher than those in the control group,with a statistically significant difference(P<0.05).The satisfaction of patients in the observation group was higher than that in the control group,with a statistically significant difference(P<0.05).Conclusion The implementation of anticipatory care for diabetic nephropathy hemodialysis patients is more conducive to reducing the risk of hypoglycemia,lowering the blood glucose value during dialysis,and improving compliance and patient satisfaction.
