血清胱抑素C、视黄醇结合蛋白与β2-微球蛋白检验在早期2型糖尿病肾病中的应用价值分析 被引量:1

摘要 目的分析血清胱抑素C(cystatin C,CysC)、视黄醇结合蛋白(retinol binding protein,RBP)与β2-微球蛋白(beta-2-microglobulin,β2-MG)在早期2型糖尿病肾病(diabetic nephropathy,DN)检验中的应用价值。方法回顾性分析2020年1月—2022年1月期间联勤保障部队第970医院威海医疗区收治的72例早期2型DN患者的临床资料,将其作为研究组;回顾性分析同期本院收治的78例单纯2型糖尿病患者的临床资料,将其作为对照组。两组患者均接受CysC、RBP、β2-MG及尿微量白蛋白(microalbuminuria,mAlb)检验。比较两组患者的CysC、RBP、β2-MG及mAlb水平,比较CysC、RBP、β2-MG单独检验与联合检验2型DN的敏感度、特异度,并分析CysC、RBP、β2-MG与早期2型DN患者mAlb的相关性。结果研究组CysC(1.26±0.12)mg/L、RBP(88.38±12.16)mg/L、β2-MG(3.85±0.50)mg/L、mAlb(90.56±8.60)mg/24 h均高于对照组的(0.35±0.10)mg/L、(50.50±10.08)mg/L、(1.60±0.40)mg/L、(25.02±2.02)mg/24 h,差异有统计学意义(t=50.041,20.831,30.543,65.397,P<0.05)。CysC、RBP、β2-MG联合检验2型DN的敏感度、特异度均高于单纯CysC、RBP、β2-MG检验,差异有统计学意义(P<0.05)。Spearman相关性分析发现,早期2型DN患者CysC、RBP、β2-MG水平与mAlb呈正相关性(r=0.685、0.558、0.608,P<0.05)。结论CysC、RBP、β2-MG在早期2型DN检验中具有临床应用意义,且联合检验的敏感度与特异度更高,为临床诊疗提供了重要的参考依据。 Objective To analyze the application value of serum cystatin C(CysC),retinol-binding protein(RBP)andβ2-microglobulin(β2-MG)in the detection of early type 2 diabetic nephropathy(DN).Methods Retrospective analysis of the clinical data of 72 patients with early stage type 2 DN admitted to the Weihai Medical Area of the 970th Hospital of the Joint Service Support Force from January 2020 to January 2022 was conducted as a study group;the clinical data of 78 patients with simple type 2 diabetes admitted to our hospital in the same period were analyzed retrospectively,and they were used as the control group.Both groups of patients received CysC,RBPβ2-MG and microalbuminuria(mAlb)test.Compare the CysC,RBPβ2-MG and mAlb levels,compared CysC,RBPβ2-MG alone and jointly tests the sensitivity and specificity of type 2 DN,and analyzes CysC,RBPβCorrelation between 2-MG and mAlb in patients with early type 2 DN.Results CysC(1.26±0.12)mg/L,RBP(88.38±12.16)mg/LβThe levels of 2-MG(3.85±0.50)mg/L and mAlb(90.56±8.60)mg/24 h in the control group were significantly higher than those in the control group(0.35±0.10)mg/L,(50.50±10.08)mg/L,(1.60±0.40)mg/L,and(25.02±2.02)mg/24 h,with a statistically significant difference(t=50.041,20.831,30.543,65.397,P<0.05).CysC,RBP,βThe sensitivity and specificity of 2-MG combined test for type 2 DN were higher than those of CysC,RBPβThe 2-MG test showed a statistically signifi cant difference(P<0.05).Spearman correlation analysis found that CysC,RBP andβ2-MG in patients with early type 2 DN were positively correlated with mAlb(r=0.685,0.558,0.608,P<0.05).Conclusion CysC,RBP andβ2-MG have clinical significance in early type 2 DN testing,and the sensitivity and specificity of combined testing are higher,providing an important reference basis for clinical diagnosis and treatment.
