王兰娣从“肺胃相关”论治胃食管反流性咳嗽

胃食管反流性咳嗽是呼吸科临床常见的慢性咳嗽之一,由于其主要表现为慢性咳嗽,多数不伴反酸等消化道症状,而导致本病容易漏诊、误诊。王兰娣主任临床工作30余年,对于本病的治疗有独到的见解,王主任从肺胃相关理论出发,认为本病病因病机在于脾失健运、湿浊内生、气滞痰阻、痰热互结、胃失和降,久延则肺脾气虚、内生瘀血。治疗予健脾和胃化湿,祛痰降逆止咳,同时强调饮食治疗及用药禁忌以防止复发。

Gastroesophageal reflux cough,one of the common chronic coughs,is prone to misdiagnosis and missed diagnosis since it mainly manifests chronic cough and is not accompanied by digestive tract symptoms such as acid reflux in most cases.Director Wang Landi with over 30 years of clinical practice has unique insights into the treatment of this disease.According to the theory of lung-stomach correlation,Director Wang believes that this disease is caused by the dysfunction of spleen in transportation,generation of internal dampness and turbidity,qi stagnation and phlegm obstruction,accumulation of combined phlegm and heat,and dysfunction of stomach in harmonizing and descending,the lingering of which leads to qi deficiency of lung and spleen and generation of internal stasis.The treatment of gastroesophageal reflux cough should focus on invigorating spleen and harmonizing stomach to resolve dampness and dispelling phlegm and descending adverse qi to relieve cough.At the same time,dietary therapy and contraindications in medication should be considered to prevent recurrence.
