摘要
目的研究围绝经期乳腺癌组织中增殖细胞核抗原ki-67与雄激素受体(AR)的表达情况以及与预后的关系。方法通过检测2017年6月至2018年6月期间的72例围绝经期乳腺癌手术标本中ki-67与AR的表达,分析两者与临床病理特征的关系,COX回归模型进行多因素分析、Kaplan-Meier法绘制生存曲线,探讨其与预后的关系。结果免疫组化染色显示,72例围绝经期乳腺癌患者中有29例(40.28%)ki-67阳性表达,42例(58.33%)AR阳性表达。不同组织学分级、分子分型的亚组病例间ki-67及AR的表达均有统计学差异(P<0.05),而在不同年龄、肿物大小、腋窝淋巴结状态亚组病例间的表达均无统计学差异(P>0.05)。COX多因素回归分析显示,乳腺癌组织中ki-67表达和分子分型是预后的独立影响因素[分别为HR=1.55,95%CI(1.01,2.34),P<0.05;HR=1.83,95%CI(1.29,2.62),P<0.05];而Kaplan-Meier生存曲线显示,乳腺癌组织中不同ki-67或AR表达状况的患者累积生存期无统计学差异(P>0.05)。结论ki-67、AR在围绝经期乳腺癌组织中表达水平不同,联合检测有助于综合评价乳腺癌的发生发展,有助于临床对乳腺癌的精准诊疗和预后评估。
Objective:To investigate the expression of ki-67 and androgen receptor(AR)in perimenopausal breast cancer and the correlation with prognosis.Methods:The expressions of ki-67 and AR were detected in surgical specimens of 72 patients with perimenopausal breast cancer from June 2017 to June 2018 in this study,and the relationship between the expressions and clinicopathological features was analyzed.COX regression model was used for multivariate analysis and Kaplan-Meier method was used to plot survival curve for exploring the relationship between the expressions of Ki-67&AR and prognosis.Results:Immunohistochemical staining showed that ki-67 was positive expression in 29 patients(40.28%)and AR was positive expression in 42 patients(58.33%)among 72 patients with perimenopausal breast cancer.There were significant differences in the expression of ki-67 and AR among the subgroups of patients with different histological grades and molecular subtypes(P<0.05),while there was no significant difference in the expression of ki-67 and AR among subgroups of patients with different age,tumor size,and axillary lymph node status(P>0.05).COX multivariate regression analysis showed that ki-67 expression[HR=1.55,95%CI(1.01,2.34),P<0.05]and molecular typing[HR=1.83,95%CI(1.29,2.62),P<0.05]in breast cancer tissues were independent prognostic factors.Kaplan-Meier survival curve showed that there was no significant difference in the cumulative survival time of patients with different expression of ki-67 or AR in breast cancer tissues(P>0.05).Conclusions:The expression levels of ki-67 are different with AR in perimenopausal breast cancer.Combined detection of expression levels of ki-67 and AR is conducive to the comprehensive evaluation of the occurrence and development of breast cancer,as well as the accurate diagnosis and treatment and prognosis evaluation of breast cancer.
