针刺鬼穴对心理生理性失眠事件相关电位N2影响的临床研究

A Clinical Study on the Effect of Acupuncture at Ghost Points on Event-Related Potential N2 of Psychophysiological Insomnia

目的:观察针刺“鬼穴”对心理生理性失眠(PPI)的事件相关电位(ERP)N2成分的影响。方法:随机选取30例健康受试者作为健康组,60例PPI患者随机分为观察组和对照组,观察组及对照组在针刺前后分别行1次PSQI、DBAS评分和N2成分的潜伏期和波幅检测,健康组则不行干预措施,在入组前行1次PSQI和DBAS评分,入组后检测1次N2的潜伏期和波幅。结果:量表方面,观察组与对照组针刺前的PSQI评分、DBAS评分差异无统计学意义(P>0.05),在针刺后均得到改善,其中观察组的针刺后评分及前后差值与对照组比较,差异具有统计学意义(P<0.05或P<0.01);脑电方面,3组针刺前后正性、负性潜伏期及正性波幅差异无统计学意义(P>0.05),观察组及对照组的针刺前负性波幅比健康组降低,差异具有统计学意义(P<0.01),针刺后均得到提升,且差异具有统计学意义(P<0.05),但观察组针刺前后的负性波幅差值与对照组比较差异具有统计学意义(P<0.05)。结论:针刺“鬼穴”治疗PPI的疗效显著,能够很好地提升N2的负性波幅,改善对睡眠的错误认知。

Objective:To observe the effect of needling‘ghost points’on N2 component of event-related potential(ERP)in treating psychophysiological insomnia(PPI).Methods:30 healthy people were randomly selected into the healthy group,60 PPI patients were randomly divided into the observation group and the control group.Scores of PSQI and DBAS,as well as latency and amplitude of N2 components were detected once before and after acupuncture in the observation group and the control group;whereas PSQI and DBAS scores were detected once before and after acupuncture in the healthy group,and the latency and amplitude of N2 were detected once after enrollment.Results:There were no statistical differences in the scores of PSQI and DBAS between the observation group and the control group before acupuncture(P>0.05),but they were improved after acupuncture,and the improvements in the observation group were more significant than those in the control group(P<0.05,P<0.01).There were no statistical differences in positive latency,negative latency and positive amplitude among the three groups before and after acupuncture(P>0.05).The negative amplitude of the observation group and the control group before acupuncture was lower than that of the healthy group,with significant statistical difference(P<0.01),and it was improved after acupuncture,with statistical difference(P<0.05).However,the difference of negative amplitude before and after acupuncture in the observation group was statistically different than that in the control group(P<0.05).Conclusion:Acupuncture at“ghost point”has a significant effect on PPI,which can well improve the negative amplitude of N2 and improve the wrong cognition of sleep.
