基于“辨经论治”理论探讨程红亮教授从“咽肾相关”治疗中风后吞咽障碍的探索

Exploration of Professor CHENG Hongliang Treating Post-Stroke Dysphagia on Basis of‘Treatment Based on Meridian Differentiation’and‘Pharynx-Kidney Correlation’

总结程红亮教授基于“辨经论治”理论从“咽肾相关”治疗中风后吞咽障碍的经验。程红亮教授认为中风后吞咽障碍病位在脑,涉及咽、喉、口和舌等部位,且十二经脉、任督二脉皆不同程度与咽相联系,据此提出“咽为经脉之所聚”理论,在治疗中风后吞咽障碍时应结合不同经脉的病证特点,以“辨经”为“论治”的核心。程红亮教授认为肾经与咽喉部关系尤为密切,“咽肾相关”实质是以任督二脉为桥梁,构建起咽、脑与肾三者之间的联系,从而形成了以任督二脉、肾经组穴为主的诊疗方案。经过不断探索,初步构建了以项部取穴+舌咽部取穴+“辨经”取穴的治疗思路,起到了通络调神、宣通咽喉之效,进一步提高了临床疗效。

This study summarized Professor CHENG Hongliang’s experience in treating post-stroke dysphagia on the basis of‘treatment based on meridian differentiation’and‘pharyngeal-Kidney correlation’.Professor CHENG Hongliang believes that the disease location of post-stroke dysphagia is in Brain,involving pharynx,larynx,mouth,tongue and other parts,and the twelve meridians and Ren and Du are all connected with pharynx to different degrees.Based on this,the theory of‘pharynx is the aggregation of meridians’is put forward,which should be combined with the characteristics of different meridians in treatment of post-stroke dysphagia,and take‘meridian differentiation’as the core of‘treatment’.Professor CHENG Hongliang believes that Kidney meridian is particularly closely related to pharyngeal,and the essence of‘pharyngeal-Kidney correlation’is to build the connection among pharynx,Brain and Kidney,with Ren meridian and Du meridian as bridges,so as to form diagnosis and treatment scheme based on the group points of Ren meridian,Du meridian and Kidney meridian.After continuous exploration,the treatment idea of selecting acupoints around neck,points around pharynx and meridian differentiation was preliminarily constructed,which plays a role in regulating Shen,dredging collateral,facilitating throat,and further improving clinical curative effect.
