老年慢性心力衰竭患者血清环状RNA同源域相互作用蛋白激酶3水平的改变

Change of serum circulating RNA homeodomain interacting protein kinase 3 in elderly patients with chronic heart failure

目的探讨慢性心力衰竭(CHF)患者血清环状RNA同源域相互作用蛋白激酶3(circHIPK3)表达情况及其诊断、预后价值。方法选取2017年9月至2021年9月于解放军联勤保障部队第九六〇医院干二科住院治疗的CHF患者200例为CHF组,其中预后良好患者158例和预后不良患者42例;同期选取心功能正常的健康体检者194例作为对照组。采用Pearson相关分析,采用ROC曲线分析血清circHIPK3诊断CHF的价值。结果与对照组比较,CHF组circHIPK3、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、N末端B型钠尿肽前体(NT-proBNP)明显升高(P<0.01),CHF组LVEF明显降低[(38.22±6.75)%vs(65.43±8.49)%,P=0.000]。CHF患者血清circHIPK3与LVEDD、LVESD、NT-proBNP呈明显正相关(P<0.01),与LVEF呈明显负相关(P<0.01)。血清circHIPK3水平诊断CHF的曲线下面积为0.878,特异性为87.1%,敏感性为73.5%。与预后良好患者比较,预后不良患者circHIPK3、LVEDD、LVESD、NT-proBNP明显升高,LVEF明显降低(P<0.01)。结论CHF患者血清circHIPK3水平升高,且与患者的预后不良相关。

Objective To investigate the expression of circHIPK3 in patients with CHF,and explore its diagnostic and prognostic values.Methods A total of 200 CHF patients hospitalized in the Second Department of Cardre􀆳s Ward of No.960 Hospital from September 2017 to September 2021 were enrolled.According to their outcomes,they were assigned into good prognosis(158 cases)and poor prognosis groups(42 cases).Meantime,194 healthy individuals with normal cardiac function were selected as the control group.Pearson correlation analysis was used to analyze the relationship between serum circHIPK3 and cardiac function indexes;the diagnostic value of serum circHIPK3 in CHF was analyzed by ROC curve analysis.Results CircHIPK3,LVEDD,LVESD and NT-proBNP were significantly higher(P<0.01),while LVEF was obviously lower in the CHF group than the control group[(38.22±6.75)%vs(65.43±8.49)%,P=0.000].Serum circHIPK3 expression in the CHF patients was obviously positively correlated with LVEDD,LVESD and NT-proBNP(P<0.01),and negatively correlated with LVEF(P<0.01).The AUC of serum circHIPK3 level in diagnosing CHF was 0.878,with a specificity of 87.1%and a sensitivity of 73.5%.CircHIPK3,LVEDD,LVESD and NT-proBNP were notably higher,while LVEF was remarkably lower in the poor prognosis group than the good prognosis group(P<0.01).Conclusion The serum level of circHIPK3 is increased in the CHF patients,which is correlated to poor prognosis of these patients.
