异位妊娠诊断中经阴道超声检查的效果分析

Analysis of the Effectiveness of Transvaginal Ultrasonography in the Diagnosis of Ectopic Pregnancy

目的探讨异位妊娠采用阴道超声检查的诊断价值。方法选取2019年2月—2020年7月菏泽市中医医院收入的异位妊娠患者155例,分别行经腹部超声检查和经阴道超声检查,比较分析两种方法的诊断准确率。结果经阴道超声符合率为98.06%高于经腹部超声的70.97%,差异有统计学意义(χ^(2)=57.036,P<0.05);155例患者中,25例(16.13%)为宫颈妊娠,23例(14.84%)为宫角妊娠,29例(18.71%)为剖宫产瘢痕妊娠,78例(50.32%)为输卵管妊娠;经阴道超声的宫角妊娠、输卵管妊娠、宫颈妊娠以及剖宫产瘢痕妊娠符合率均明显高于经腹部超声,差异有统计学意义(P<0.05);同时,经腹部超声的胚芽、假孕囊、附件区包块、盆腔积液以及原始心管搏动符合率均高于经阴道超声,差异有统计学意义(P<0.05)。结论临床上运用阴道超声检查诊断异位妊娠,不仅可以提高检出率,明确异位妊娠部位,有助于制订针对性治疗方案,具有一定的推广运用价值。

Objective To explore the diagnostic value of ectopic pregnancy using transvaginal ultrasonography.Methods 155 patients with ectopic pregnancy who were admitted by Heze Traditional Chinese Medicine Hospital from February 2019 to July 2020 were selected to undergo transabdominal ultrasound and transvaginal ultrasound respectively,and the diagnostic accuracy of the two methods was compared and analyzed.Results The coincidence rate of transvaginal ultrasound was 98.06%higher than that of transabdominal ultrasound(70.97%),and the difference was statistically significant(χ^(2)=57.036,P<0.05).Of 155 patients,25(16.13%)were cervical pregnancy,23(14.84%)were cornual pregnancy,29(18.71%)were cesarean scar pregnancy,and 78(50.32%)were tubal pregnancy.The coincidence rate of cornual pregnancy,tubal pregnancy,cervical pregnancy and cesarean scar pregnancy by transvaginal ultrasound were significantly higher than those by transabdominal ultrasound,the difference was statistically significant(P<0.05).At the same time,the compliance rate of embryo,pseudopregnancy sac,adnexal mass,pelvic effusion and original cardiac pulsation by transvaginal ultrasound were significantly higher than those by transvaginal ultrasound,the difference was statistically significant(P<0.05).Conclusion The use of vaginal ultrasonography in clinical diagnosis of ectopic pregnancy not only improves the detection rate and clarifies the identify the location of ectopic pregnancy,which helps to develop a targeted treatment plan and has certain promotion value.
