超声征象评分对胎盘植入诊断的准确率分析

Analysis of the Accuracy of Ultrasound Signs Score in Diagnosing Placenta Implantation

目的分析超声征象评分诊断胎盘植入的准确率。方法选择2020年7月—2022年6月期间于江苏省连云港市妇幼保健院超声科进行产前检查的86例孕妇为研究对象,根据临床以及病理学检查结果(金标准),以是否存在胎盘植入分为植入组(观察组,n=43)和未植入组(对照组,n=43),两组孕妇均通过产前超声检查进行超声征象评分,对比两组患者的超声征象及评分、诊断准确率。结果观察组超声征象评分为(4.26±1.15)分,对照组评分为(0.88±0.13)分,差异有统计学意义(t=19.151,P<0.05)。观察组产妇胎盘增厚、子宫肌层最薄处<2 mm、宫颈膨大以及胎盘后间隙部分或完全消失的发生率高于对照组,差异有统计学意义(χ^(2)=18.860、9.161、9.241、22.744,P<0.05)。超声征象评分诊断胎盘植入具有较高的特异性、灵敏度,与临床病理检查具有较高的一致性(Kappa=0.930)。结论通过超声征象评分诊断胎盘植入具有极高的准确性和敏感度,能够为临床诊疗工作的开展提供支持。

Objective To analyze the accuracy of ultrasound sign score in diagnosis of placental implantation.Methods 86 pregnant women who underwent prenatal examination in the Ultrasound Department of Lianyungang Maternal and Child Health Hospital in Jiangsu Province from July 2020 to June 2022 were selected as the study subjects.According to the clinical and pathological examination results(gold standard),they were divided into implantation group(observation group,n=43)and non-inimplantation group(control group,n=43)for the presence or absence of placenta accreta.Both groups of pregnant women were scored by prenatal ultrasound,and the ultrasound signs,scores and diagnostic accuracy of the two groups were compared.Results The ultrasound sign score of the observation group was(4.26±1.15)points,and the score of the control group was(0.88±0.13)points,the difference was statistically significant(t=19.151,P<0.05).The incidence of placental thickening,the thinnest part of uterine myometrium<2 mm,cervical enlargement and partial or complete disappearance of posterior placental space in the observation group were higher than those in the control group,the difference was statistically significant(χ^(2)=18.860,9.161,9.241,22.744,P<0.05).The ultrasound sign score has high specificity and sensitivity in diagnosis of placental implantation,and has high consistency with clinical pathological examination(Kappa=0.930).Conclusion The diagnosis of placental implantation by ultrasound sign scoring has high accuracy and sensitivity,which can provide support for clinical diagnosis and treatment.
