抗菌药物辅助应用对急性阑尾炎腹腔镜手术患者的影响

Effect of Adjuvant Application of Antibiotics on Patients Undergoing Laparoscopic Operation for Acute Appendicitis

目的探究抗菌药物辅助应用对急性阑尾炎腹腔镜手术患者的影响。方法选择宜兴市第二人民医院2019年11月—2022年3月期间100例急性阑尾炎腹腔镜手术患者,以随机数表法分为A组(50例,头孢地嗪静脉给药)、B组(50例,头孢地嗪静脉给药+头孢地嗪切口局部给药),比较两组切口感染发生率、细菌阳性检出率、药物不良反应发生率。结果B组浅表手术切口感染率为4.00%,较A组的16.00%低,差异有统计学意义(χ^(2)=4.000,P<0.05);B组总手术切口感染率较A组低,差异有统计学意义(χ^(2)=6.601,P<0.05);B组大肠杆菌阳性检出率较A组低,差异有统计学意义(χ^(2)=4.396,P<0.05)。结论对急性阑尾炎腹腔镜手术患者抗菌药物辅助治疗中,应用头孢地嗪静脉联合切口局部给药治疗,可降低浅表手术切口及总感染发生率,降低大肠杆菌阳性检出率,且并未增加抗生素不良反应发生率。

Objective To explore the effect of adjuvant application of antibiotics on patients undergoing laparoscopic operation for acute appendicitis.Methods A total of one hundred patients undergoing laparoscopic surgery for acute appendicitis in the Yixing Second People's Hospital from November 2019 to March 2022 were selected and divided into group A(fifty patients receiving intravenous administration of cefodizine)and Group B(fifty patients receiving intravenous administration of cefodizine+local incision administration of cefodizine)by random number table method.The incidence of incision infection,positive bacterial detection rate and adverse drug reaction rate were compared between the two groups.Results The superficial incision infection rate in group B was 4.00%,which was lower than that in group A(16.00%),and the difference was statistically significant(χ^(2)=4.000,P<0.05).The total incision infection rate in group B was lower than that in group A,and the difference was statistically significant(χ^(2)=6.601,P<0.05).The positive detection rate of Escherichia coli in group B was lower than that in group A,and the difference was statistically significant(χ^(2)=4.396,P<0.05).Conclusion For patients undergoing laparoscopic surgery for acute appendicitis,the application of cefodizine intravenous combined incision local administration can reduce the incidence of superficial surgical incision and total infection,reduce the positive detection rate of Escherichia coli,and does not increase the incidence of antibiotic adverse reactions.
