剖宫产分娩与自然分娩对瘢痕子宫再妊娠孕妇母婴结局的影响

Effect of cesarean section delivery and vaginal delivery on maternal and infant outcomes of cicatricial uterus repregnancy

目的 研究剖宫产分娩与自然分娩对瘢痕子宫再妊娠孕妇母婴结局的影响。方法 选取郑州大学第一附属医院2019年11月—2022年11月收治的瘢痕子宫再妊娠孕妇86例作为此次研究对象,以不同分娩方式分组。对照组(共43例)采取剖宫产分娩,观察组(共43例)采取阴道分娩,观察对比两组孕妇分娩情况、新生儿情况、并发症及产程。结果 (1)观察组孕妇产时及产后2 h、24 h的出血量较对照组显著降低,住院时间较对照组显著缩短,P<0.05。(2)观察组新生儿体重、Apgar评分及产程与对照组相比,差异无统计学意义,P>0.05。(3)观察组孕妇并发症的发生率为2.33%,较对照组的16.28%显著降低,P<0.05。(4)观察组新生儿并发症的发生率为4.65%,较对照组的18.60%显著降低,P<0.05。结论 经全面评估并排除禁忌指征后,相较剖宫产分娩,自然分娩可显著降低孕妇及新生儿并发症的发生率,有效改善母婴结局。

Objective To study the effects of cesarean section and natural delivery on maternal and infant outcomes of pregnant women with cicatricial uterus.Methods Totally 86 pregnant women with cicatricial uterus who received treatment in our hospital from November 2019 to November 2022 were selected as subjects of this study,and grouped by difference modes of delivery.The control group(43 cases)was delivered by cesarean section,and the observation group(43 cases)was delivered by vaginal delivery.Results①The blood loss at the time of delivery and 2h and 24h after delivery in the observation group was significantly lower than that in the control group,and the length of hospitalization was significantly shorter than that in the control group,P<0.05.②There were no significant differences in neonatal weight,Apgar score and labor process between the observation group and the control group(P>0.05).③The incidence of complications in the observation group was 2.33%,significantly lower than that in the control group(16.28%),P<0.05.④The incidence of neonatal complications in the observation group was 4.65%,significantly lower than that in the control group(18.60%),P<0.05.Conclusion After comprehensive evaluation and exclusion of contraindicated indications,compared with cesarean section,natural delivery can significantly reduce the incidence of maternal and neonatal complications and effectively improve maternal and infant outcomes.
