腹腔镜下微创手术治疗胃十二指肠穿孔临床疗效

Clinical Effect of Minimally Invasive Laparoscopic Surgery on Gastroduodenal Perforation

目的分析腹腔镜下微创手术治疗胃十二指肠穿孔效果。方法随机选取2020年3月—2022年3月东营市第二人民医院普外科的62例胃十二指肠穿孔患者,按照双盲法分组为对照组(31例)行开腹修补手术,观察组(31例)行腹腔镜下微创手术,对比两组临床指标、并发症发生率、治疗有效率、生活质量评分。结果观察组手术时间、术中出血量、下床活动时间、住院时间、肛门排气时间、放置引流管时间、恢复消化道功能时间(39.26±18.27)min、(82.43±6.56)mL、(16.23±4.86)h、(6.13±1.85)d、(58.92±6.37)h、(1.57±0.75)d、(1.94±0.43)d均短于对照组,差异有统计学意义(t=8.510、10.173、11.256、6.596、11.626、8.326、8.881,P<0.05);观察组治疗有效率、生活质量评分均优于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论胃十二指肠穿孔患者在腹腔镜下进行微创手术效果显著,和生活质量效果佳。

Objective To analyze the effect of minimally invasive laparoscopic surgery on gastroduodenal perforation.Methods Randomly selected 62 patients with gastroduodenal perforation from the General Surgery Department of Dongying Second People's Hospital from March 2020 to March 2022,according to the double blind method,the control group(31 cases)underwent open repair surgery,while the observation group(31 cases)underwent laparoscopic minimally invasive surgery.The clinical indicators,complication rate,treatment efficiency and quality of life score were compared between the two groups.Results Operation time,intraoperative blood loss,ambulation time,hospital stay,anal exhaust time,drainage tube placement time,digestive tract function recovery time(39.26±18.27)min,(82.43±6.56)mL,(16.23±4.86)h,(6.13±1.85)d,(58.92±6.37)h,(1.57±0.75)d,(1.94±0.43)d in the observation group were shorter than the control group,the difference was statistically significant(t=8.510,10.173,11.256,6.596,11.626,8.326,8.881,P<0.05).The effective rate and quality of life score in the observation group were better than those of control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Gastroduodenal perforation in patients with minimally invasive laparoscopic surgery results in significant results and better quality of life.
