桃红四物汤联合低分子肝素钠防止老年股骨转子间骨折术后深静脉血栓效果分析

Analysis of the Effect of Taohong Siwu Decoction Combined with Low Molecular Heparin Sodium to Prevent Deep Vein Thrombosis after Intertrochanteric Fractures of Femur in the Elderly

目的分析老年股骨转子间骨折术后采用桃红四物汤联合低分子肝素钠防止深静脉血栓的价值。方法简单随机选取2017年1月—2021年12月兴化市中医院治疗的老年股骨转子间骨折患者80例作为研究对象,并按随机抽样法分两组(每组40例),对照组术后给予低分子肝素钠治疗,观察组术后给予桃红四物汤联合低分子肝素钠治疗,对比;两组临床疗效、凝血四项、不良反应率、血栓发生率、血液流变学指标、关节功能。结果观察组临床疗效(95.00%)高于对照组(80.00%),差异有统计学意义(χ^(2)=4.114,P<0.05);观察组凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)高于对照组,血浆D-二聚体(D-D)低于对照组,差异有统计学意义(P<0.05);观察组不良反应率与对照组相比,差异无统计学意义(P>0.05);观察组血栓发生率(5.00%)低于对照组(22.50%),差异有统计学意义(P<0.05);观察组高切、中切、低切、血浆黏度低于对照组,差异有统计学意义(P<0.05);观察组术后4周、12周、24周关节功能评分高于对照组,差异有统计学意义(P<0.05)。结论选择桃红四物汤联合低分子肝素钠可有效预防老年股骨转子间骨折术后深静脉血栓,值得借鉴。

Objective To analyze the value of using Taohong Siwu decoction combined with low molecular heparin so-dium to prevent deep vein thrombosis after intertrochanteric fractures of femur in the elderly.Methods 80 elderly pa-tients with intertrochanteric fractures of femur treated in Xinghua Hospital of Traditional Chinese Medicine from Janu-ary 2017 to December 2021 were simple randomly selected as study subjects and divided into 2 groups(40 cases in each group)according to random sampling method.The control group was treated with low molecular heparin sodium after surgery,and the observation group was treated with Taohong Siwu decoction combined with low-molecular hepa-rin sodium after surgery.The clinical efficacy,coagulation four items,adverse reaction rate,thrombosis rate,blood rhe-ology index and joint function of the two groups were compared.Results The clinical effect of the observation group(95.00%)was higher than that of the control group(80.00%),and the difference was statistically significant(χ^(2)=4.114,P<0.05).prothrombin time(PT),trombin time(TT),activeated partial thromboplasting time(APTT)of the observation group were higher than those of the control group.Plasma D-Dimer(D-D)was lower than that of control group,and the difference was statistically significant(P<0.05).There was no statistically statistically significant difference in the adverse reaction rate between the observation group and the control group(P>0.05).The incidence of thrombus in ob-servation group(5.00%)was lower than that in control group(22.50%),and the difference was statistically significant(P<0.05).High cut,medium cut,low cut and plasma viscosity in the observation group were lower than those in the con-trol group,the difference was statistically significant(P<0.05).The joint function score of the observation group was higher than that of the control group at 4 weeks,12 weeks and 24 weeks after surgery,and the difference was statisti-cally significant(P<0.05).Conclusion The selection of Taohong Siwu decoction combined with low molecular heparin sodium can effectively prevent deep vein thrombosis after intertrochanteric fractures of femur in the elderly.
