超声乳化白内障吸除术治疗硬核白内障的效果及对术后干眼症的影响

Effectiveness of Ultrasonic Emulsion Cataract Aspiration for Sclerotic Cataract and Effects on Postoperative Dry Eye Syndrome

目的评估硬核白内障患者接受超声乳化白内障吸除术治疗的临床效果及术后并发症情况。方法方便选择2022年1—8月在泗阳县中医院接受手术治疗的硬核白内障患者116例作为研究对象,将其分为对照组(58例)与观察组(58例),对照组采取小切口白内障切除术,观察组采取超声乳化白内障吸除术,比较两组患者术前及术后1个月视力及眼压变化,测定两组患者术前及术后1个月泪膜破裂时间及泪腺功能变化,评估比较两组患者治疗前后干眼症状差异以及术后不良反应总发生率。结果术前两组患者视力、眼压结果比较,差异无统计学意义(P>0.05),观察组患者术后裸眼视力(0.85±0.12)高于对照组,散光度数(0.40±0.11)D低于对照组,眼压水平(15.12±1.07)mmHg低于对照组,差异有统计学意义(t=11.696、18.783、14.495,P<0.05)。同时观察组患者泪腺破裂时间长于对照组,泪液分泌量高于对照组,差异有统计学意义(P<0.05)。观察组患者干眼症状评分以及术后不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论对于硬核白内障的手术治疗,超声乳化白内障吸除术能够有效改善患者视力,并减少术后干眼症等并发症的发生概率。

Objective To evaluate the clinical outcomes and postoperative complications of patients with sclerotic cataract treated with ultrasonic emulsion cataract aspiration.Methods 116 patients with sclerotic cataract who underwent surgical treatment at Siyang County Hospital of Traditional Chinese Medicine from January to August 2022 were conveniently selected as the study subjects,and were divided into a control group(58 cases)and an observation group(58 cases).The control group took small incision cataract resection,and the surgical protocol of the observation group took ultrasonic emulsion cataract aspiration.To compare the changes in visual acuity and intraocular pressure before and 1 month after surgery,to determine the time of tear film rupture and changes in lacrimal gland function before and 1 month after surgery,and to evaluate and compare the differences in dry eye symptoms before and after treatment,as well as the total incidence of postoperative adverse reactions between the two groups of patients.Results There was no statistically significant difference between the two groups of patients´visual acuity and intraocular pressure(P>0.05).The postoperative naked eye visual acuity of patients in the observation group(0.85±0.12)was higher than that of the control group,the astigmatism(0.40±0.11)D was lower than that of the control group,and the intraocular pressure level(15.12±1.07)mmHg was lower than that of the control group,and the difference was statistically significant(t=11.696,18.78314.495,P<0.05).At the same time,patients in the observation group had longer tear gland rupture time and higher tear secretion than the control group,and the difference was statistically significant(P<0.05).The dry eye symptom scores of patients and the overall incidence of postoperative adverse reactions was lower in the observation group than in the control group,and the difference was statistically significant(P<0.05).Conclusion For the surgical treatment of sclerotic cataract,ultrasonic emulsion cataract aspiration can effectively improve patients´visual acuity and reduce the incidence of postoperative complications such as dry eye.
