集束化护理模式在结直肠癌PICC置管患者中的应用及对患者自我管理效能的影响

Application of Bundled Nursing Mode in Colorectal Cancer Patients with PICC Catheterization and Its Influence on Self-management Efficacy of Patients

目的探究集束化护理模式在结直肠癌PICC置管患者中应用及对患者自我管理效能的影响。方法回顾性选取2020年1月—2021年12月期间于福建省漳州市医院进行PICC置管的131例结直肠癌患者作为研究对象进行回顾性分析,其中2020年1—12月置管的65例接受常规护理,列入对照组。2021年1—12月置管的66例患者接受集束化护理,列入观察组。采用肿瘤患者PICC自我管理能力量表评估两组患者自我管理效能,采用心理弹性量表(CD-RISC)评估患者的心理状态,采用生活质量量表评估患者生活水平。结果护理后,观察组肿瘤患者PICC自我管理能力评分为(154.68±11.34)分,明显高于对照组(137.26±10.87)分,差异有统计学意义(t=8.973,P<0.05)。观察组CD-RISC评分为(83.23±6.65)分,明显高于对照组的(69.96±6.38)分,差异有统计学意义(P<0.05)。观察组SF-36中的躯体疼痛、生理职能、社会功能、一般健康评分明显高于对照组,差异有统计学意义(P<0.05)。结论对结直肠癌PICC置管患者实施集束化护理可以改善患者心理状态,提高自我管理效能,改善患者生活质量。

Objective To explore the application of cluster nursing mode in patients with colorectal cancer PICC catheterization and its influence on selfmanagement efficacy of patients.Methods A retrospective analysis was conducted on 131 colorectal cancer patients who underwent PICC catheterization at Zhangzhou Hospital of Fujian Province from January 2020 to December 2021.Among them,65 patients who underwent PICC catheterization from January to December 2020 received routine care and were included in the control group.From January to December 2021,66 patients placed in catheters received cluster care and were included in the observation group.The PICC Self Management Ability Scale for Cancer Patients was used to evaluate the selfmanagement efficacy of two groups of patients,the Psychological Resilience Scale(CDRISC)was used to evaluate the patient´s psychological state,and the Quality of Life Scale was used to evaluate the patient´s living standards.Results After nursing,the PICC selfmanagement ability score of tumor patients in the observation group was(154.68±11.34)points,significantly higher than that of the control group(137.26±10.87)points,and the difference was statistically significant(t=8.973,P<0.05).The CDRISC score of the observation group was(83.23±6.65)points,significantly higher than the control group´s(69.96±6.38)points,and the difference was statistically significant(P<0.05).The body pain,physiological function,social function,and general health scores of SF36 in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The implementation of bundled nursing care for colorectal cancer patients with PICC catheters can improve the psychological state of patients,improve selfmanagement efficiency,and improve the quality of life of patients.
