医护一体化康复管理对股骨颈骨折患者康复锻炼依从性及髋关节功能的影响

Effect of Integrated Rehabilitation Management of Doctors and Nurses on Compliance of Rehabilitation Exercise and Hip Joint Function of Patients with Femoral Neck Fracture

目的探究在股骨颈骨折患者中实施医护一体化康复护理对康复锻炼依从性、髋关节功能的影响。方法纳入本院2019年5月至2022年5月收治的106例股骨颈骨折患者,按随机数字表法分为对照组和观察组,各53例。对照组采取常规护理,观察组则开展医护一体化康复护理,均持续护理3个月。比较两组康复锻炼依从性、髋关节功能、生活质量、并发症。结果观察组康复锻炼依从性(94.34%)较对照组(77.36%)高,差异有统计学意义(P<0.05);对比两组护理前髋关节功能、生活质量,差异无统计学意义(P>0.05);观察组护理后髋关节功能评分表(Harris)中疼痛评分(35.42±2.21)分、功能评分(37.16±2.76)分、关节活动度评分(3.49±0.26)分、肢体畸形评分(2.96±0.25)分均较对照组高,欧洲五维健康量表(EQ-5D)中行动能力评分(79.38±4.26)分、疼痛或不舒服评分(78.12±4.66)分、自我照护能力评分(80.98±5.54)分、焦虑或抑郁评分(82.22±5.01)分、日常活动能力评分(78.54±4.11)分均较对照组高,差异有统计学意义(P<0.05);与对照组(18.87%)并发症发生率相比,观察组(3.77%)较低,差异有统计学意义(P<0.05)。结论在股骨颈骨折患者中实施医护一体化康复护理能够提高康复锻炼依从性,减少并发症,改善髋关节功能,进而提升生活质量。

Objective To explore the effect of medical and nursing integrated rehabilitation nursing on compliance of rehabilitation exercise and hip joint function in patients with femoral neck fracture.Methods A total of 106 patients with femoral neck fracture admitted to our hospital from May 2020 to May 2022 were randomly divided into the control group and the observation group,with 53 patients in each group.The control group received routine nursing,while the observation group received integrated medical and nursing rehabilitation nursing,which lasted for 3 months.The compliance of rehabilitation exercise,hip function,quality of life and complications were compared between the two groups.Results The compliance of rehabilitation exercise in the observation group(94.34%)was higher than that in the control group(77.36%),and the difference was statistically significant(P<0.05).There was no significant difference in hip function and quality of life between the two groups before nursing(P>0.05).After nursing,the pain score(35.42±2.21),function score(37.16±2.76),joint range of motion score(3.49±0.26),limb deformity score(2.96±0.25)in the observation group were higher than those in the control group.The action ability score(79.38±4.26),pain or discomfort score(78.12±4.66),self-care ability score(80.98±5.54)in the European five dimensional health scale(EQ-5D),the scores of anxiety or depression(82.22±5.01)and daily activity ability(78.54±4.11)were significantly higher than those of the control group(P<0.05).Compared with the control group(18.87%),the incidence of complications in the observation group(3.77%)was lower,with a statistically significant difference(P<0.05).Conclusion The implementation of medical and nursing integrated rehabilitation nursing in patients with femoral neck fracture can improve the compliance of rehabilitation exercise,reduce complications,improve hip joint function,and then improve the quality of life.
