三种不同动灸手法激发变应性鼻炎患者热敏灸感效率与灸效相关性研究

Moxibustion Efficiency and Heat-sensitive Moxibustion Firing Rate Induced by Three Different Moxibustion Techniques in Patients with Allergic Rhinitis:a Correlation Study

目的:观察三种不同动灸手法下,变应性鼻炎(AR)患者热敏灸感激发效率与灸疗的差异性,探讨其相关性。方法:纳入90例AR患者,采用随机数字表法,随机分为三组,分别是回旋灸组(30例)、雀啄灸组(30例)和循经往返灸组(30例)。受试者均以印堂、神庭、迎香、肺俞等穴位为高发穴区探寻部位,分别予以回旋灸、雀啄灸、循经往返灸为探查方式,确定热敏灸穴位后进行系统治疗,每日1次,10次一疗程,共2个疗程。评价各组患者的灸感情况(灸感出现的时间、持续时间、灸感类型构成),比较各组患者治AR症状体征评分、鼻结膜炎生存(RQLQ)评分及其临床疗效。结果:雀啄灸组的灸感出现时间较回旋灸组和循经往返灸组更早(P<0.05),灸感持续时间更长(P<0.05);三组灸感类型没有差异(P>0.05);雀啄灸组RQLQ评分、AR症状体征评分更低(P<0.05),回旋灸组和循经往返灸组没有差异(P>0.05);回旋灸组愈显率为86.67%,雀啄灸组愈显率为93.33%,循经往返灸组愈显率为86.67%。雀啄灸组的愈显率高于回旋灸组、循经往返灸组(P<0.05)。结论:在以热敏灸疗法为干预措施治疗AR时,可施以雀啄灸的动灸手法以更高效地激发热敏灸感,维持更长时间的灸感及取得更好的临床疗效。

Objective:To observe the difference between the efficiency of thermal moxibustion and moxibustion therapy in patients with allergic rhinitis(AR)under three different moxibustion techniques,and to explore the correlation. Methods:90 CASES of AR patients were enrolled and randomly divided into three groups by random number table method,which were cyclotron moxibustion group(30 cases),sparrow pecking moxibustion group(30 cases)and round-channel moxibustion group(30 cases).The subjects used Yintang,Shenting,Yingxiang,Feishu and other acupoints as the high-incidence point areas to explore the sites,respectively,rotation moxibustion,finch pecking moxibustion,round-trip moxibustion as the exploration method,determined the heat sensitive moxibustion acupoints for systematic treatment,once a day,10 times a course of treatment,a total of 2 courses.The moxibustion sensation(occurrence time,duration and type composition of moxibustion sensation)was evaluated in each group,and the AR symptom and sign score,rhinoconjunctivitis survival(RQLQ)score and clinical efficacy were compared in each group. Results:The occurrence time of moxibustion feeling in the finch moxibustion group was earlier than that in the roundabout moxibustion group and the round-trip moxibustion group(P<0.05),and the duration of moxibustion feeling was longer(P<0.05).There was no difference in moxibustion sensation types among the three groups(P>0.05).RQLQ score and AR symptom and sign score were lower in the finch moxibustion group(P<0.05),and there was no difference between the cyclotron moxibustion group and the round-passage moxibustion group(P>0.05).The obvious rate was 86.67%in the cyclotron moxibustion group,93.33%in the finch moxibustion group and 86.67%in the round-trip moxibustion group.The rate of healing in sparrow moxibustion group was higher than that in cyclotron moxibustion group and round-trip moxibustion group(P<0.05). Conclusion:In the treatment of AR with heat sensitive moxibustion therapy as an intervention measure,finch peck moxibustion can be used to stimulate heat sensitive moxibustion feeling more effectively,maintain the moxibustion feeling for a longer time and achieve better clinical efficacy.
