基于数据挖掘技术探讨针灸治疗腰椎间盘突出症急性期的选穴规律

Study on Acupoint Selection Rules of Acupuncture in the Treatment of Acute Lumbar Disc Herniation Based on Data Mining Technology

目的:通过数据挖掘技术探讨腰椎间盘突出症急性期的临床选穴规律。方法:检索CNKI、WF、CBM、VIP、PubMed从建库至2022年5月10日收录的针灸治疗腰椎间盘突出症急性期的临床研究文献,用Excel 2010软件建立针灸处方数据库并进行描述性分析,运用SPSS Modeler 18.0及SPSS Statistics 26.0软件对数据进行关联规则分析与聚类分析。结果:最终纳入文献81篇,提取处方98条,共涉及腧穴59个,总频次为688次,频次前5位的腧穴分别为委中、环跳、腰夹脊、阳陵泉、肾俞,频次最高的经络和部位分别为足太阳膀胱经与下肢部。通过关联规则分析得到以委中、环跳、腰夹脊、阳陵泉、肾俞为核心的处方。对频次≥10的腧穴进行聚类分析,得到4个有效聚类群,类1:环跳、阳陵泉、委中、腰夹脊、秩边、悬钟、承山、昆仑、承扶、殷门;类2:阿是穴、次髎;类3:关元俞、气海俞、大肠俞:类4:腰阳关、命门、肾俞、风市。结论:针灸治疗腰椎间盘突出症急性期临床取穴以阳经为主,重视远近配穴及特定穴的应用,体现了“腧穴所在,主治所在”、“经脉所过,主治所及”的基本规律。

Objective:The rules of acupoint selection of acupuncture in the treatment of Acute lumbar disc herniation was discussed by data mining technology.Methods:The clinical research literatures on acupuncture treatment of acute lumbar disc herniation collected by CNKI,WF,CBM,VIP and PubMed from the establishment of the database to May 10,2022 were searched.The acupuncture prescription database was established by Excel2010 software and descriptive analysis was carried out.SPSS Modeler 18.0 and SPSS Statistics 26.0 software were used to analyze the association rules and cluster analysis.Results:Finally,81 articles were included and 98 prescriptions were extracted,involving 59 acupoints with a total frequency of 688.The top 5 acupoints were Weizhong,Huantiao,waist Jiaji,Yanglingquan and Shenshu,respectively.The meridians and parts with the highest frequency were the foot-sun bladder meridian and the lower limb.Through the analysis of association rules,the prescriptions with Weizhong,Huantiao,waist Jiaji,Yanglingquan and Shenshu as the core are obtained.Based on the cluster analysis of acupoints with frequency≥10,four effective clusters can be obtained.Class 1:Huantiao,Yanglingquan,Weizhong,waist Jiaji,Jiabian,Xuanzhong,Chengshan,Kunlun,Chengfu,Yinmen;Class 2:Ashi acupoint,Ciliu;Class 3:Guanyuanshu,Qihai Shu,Changshu:class 4:Yaoyangguan,Mingmen,Shenshu,Fengshi.Conclusion:Yang meridian is the main acupoint in acupuncture treatment of acute lumbar disc herniation,and attention is paid to the application of far and near acupoints and specific acupoints,which embodies the basic law of“where the acupoints are,where the indications areand”“where the meridians pass,where the indications are”.
