炔雌醇环丙孕酮、吡格列酮治疗2型糖尿病伴PCOS患者的临床疗效

Clinical Efficacy of Ethinylestradiol Cyproterone and Pioglitazone in Treatment of Type 2 Diabetes Mellitus with PCOS

目的探讨炔雌醇环丙孕酮、吡格列酮治疗2型糖尿病伴多囊卵巢综合征(polycystic ovarian syndrome,PCOS)患者的临床疗效。方法选取2018年1月—2021年12月期间厦门医学院附属第二医院收治的80例2型糖尿病伴PCOS患者,根据患者的治疗方式差异将其分入观察组和对照组,每组40例。对照组患者采用炔雌醇环丙孕酮+二甲双胍治疗,观察组患者采用炔雌醇环丙孕酮+吡格列酮治疗。比较两组治疗前后血糖水平改善情况以及治疗半年后排卵率、受孕率情况。结果治疗前,两组患者空腹血糖、餐后2 h血糖、三酰甘油、总胆固醇水平对比,差异无统计学意义(P>0.05);治疗后,两组各项指标均显著改善,且观察组显著优于对照组,差异有统计学意义(P<0.05);观察组治疗半年后排卵率、受孕率均显著高于对照组,差异有统计学意义(P<0.05)。结论炔雌醇环丙孕酮、吡格列酮治疗2型糖尿病伴PCOS患者具有良好的临床疗效,能显著促进机体血糖水平改善。

Objective To investigate the clinical efficacy of ethinylestradiol cyproterone and pioglitazone in the treatment of type 2 diabetes mellitus with polycystic ovarian syndrome(PCOS).Methods A total of 80 patients with type 2 diabetes mellitus and PCOS admitted to the Second Affiliated Hospital of Xiamen Medical College from January 2018 to December 2021 were selected and divided into observation group and control group according to the differences in treatment methods,with 40 cases in each group.Control group was treated with ethinylestradil cyproterone+metformin,observation group was treated with ethinylestradil cyproterone+pioglitazone.The improvement of blood glucose level before and after treatment,ovulation rate and conception rate after six months of treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in fasting blood glucose,2-hour postprandial blood glucose,triglycerides,and total cholesterol levels between the two groups of patients(P>0.05);after treatment,all indicators in both groups were significantly improved,and the observation group was significantly better than those in the control group,the difference was statistically significant(P<0.05).After six months of treatment,the ovulation rate and pregnancy rate in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion Ethinylestradiol cyproterone and pioglitazone have good clinical efficacy in the treatment of type 2 diabetes patients with PCOS,and can significantly improve the blood glucose level.
