患者参与式护理在膝关节置换术后早期功能锻炼中的效果

Effects of patient participation nursing on early functional exercise after knee arthroplasty

目的探讨患者参与式护理在膝关节置换术后早期功能锻炼干预中的实施效果。方法将2022年3月-6月收治的38例膝关节置换术患者设为对照组,2022年7月-10月收治的40例膝关节置换术患者设为干预组。对照组实施常规护理,观察组在常规护理的基础上实施患者参与式护理。比较2组膝关节功能评分、锻炼依从性、并发症、患者满意度及住院时间。结果观察组功能锻炼依从性、患者满意度均好于对照组(t=11.973,P<0.001;t=4.199,P<0.001);住院时间短于对照组(t=-2.071,P=0.043);静脉血栓栓塞、患肢肿胀发生率均低于对照组(χ^(2)=4.347,P=0.037;χ^(2)=4.253,P=0.039)。2组膝关节功能评分在时间效应、组间效应、交互效应上比较,差异均有统计学意义(F组间=7.101,P组间<0.001;F时间=203.717,P时间<0.001;F交互=2.761,P交互<0.001)。结论患者参与式护理能够有效提高膝关节置换术后患者膝关节功能评分及患者功能锻炼依从性,降低并发症的发生率,缩短住院时间,提高患者满意度。

Objective To explore the effect of patient participation nursing on early functional exercise after knee arthroplasty.Methods Thirty-eight knee arthroplasty patients admitted between March and June 2022 were selected into the control group,while another 40 counterparts admitted between July and October 2022 were chosen into the observation group.Both groups received routine nursing,while the observation group was additionally provided with patient participation nursing.The knee joint function score,compliance,satisfaction and hospital stay were compared between the two groups.Results The compliance and satisfaction of the observation group were significantly better than that of the control group(t=11.973,P<0.001;t=4.199,P<0.001).However,the length of hospital stay was shorter than that of the control group(t=-2.071,P=0.043),and the incidence of venous thrombus embolism and swelling rate of the affected limb were significantly lower than that of the control group(χ^(2)=4.347,P=0.037;χ^(2)=4.253,P=0.039).Moreover,there were significant differences in knee function scores between the two groups in terms of time,intergroup and interaction effects(F groups=7.101,P groups<0.001;F time=203.717,P time<0.001;F interaction=2.761,P interaction<0.001).Conclusion Patient participation nursing program can effectively improve the functional score and compliance of patients after knee arthroplasty,lower the incidence of complications,shorten hospital stay and promote patient satisfaction.
