彩色多普勒超声对甲状腺结节的临床诊断价值研究

Clinical diagnostic value of color Doppler ultrasound in thyroid nodules

目的探讨甲状腺结节诊断中采用彩色多普勒超声的价值。方法遵循随机原则,选择2020年6月~2022年6月在我院确诊为甲状腺结节的100例患者,均实施彩色多普勒超声检查,以病理学检查结果为金标准,分析彩色多普勒超声诊断准确率、良恶性结节的超声特征、血流动力学参数。结果彩色多普勒超声检查准确率与病理检查结果对比无统计学意义(P>0.05)。良性结节超声特征中微钙化、无声晕、病灶不规则、低回声、边界不清晰等发生率低于恶性结节(P<0.05)。良性结节血流动力参数中搏动指数、阻力指数、结节最大流速低于恶性结节,结节最小流速高于恶性结节(P<0.05)。结论在对甲状腺结节诊断中采用彩色多普勒超声可结合血流动力学参数鉴别良恶性肿瘤,为医师制定手术方案提供参考。

Objective To explore the value of color Doppler ultrasound in the diagnosis of thyroid nodules.Methods Following the principle of randomization,100 patients with thyroid nodules diagnosed in our hospital from June 2020 to June 2022 were selected for color Doppler ultrasound examination.The diagnostic accuracy of color Doppler ultrasound,ultrasound characteristics of benign and malignant nodules,and hemodynamic parameters were analyzed based on the results of pathological examination.Results There was no significant difference between the accuracy of color Doppler ultrasound and the results of pathological examination(P>0.05).The incidence of microcalcification,silent halo,irregular focus,low echo and unclear boundary in benign nodules was lower than that in malignant nodules(P<0.05).In the hemodynamic parameters of benign nodules,pulsation index,resistance index and maximum flow velocity of nodules were lower than those of malignant nodules,while the minimum flow velocity of nodules was higher than that of malignant nodules(P<0.05).Conclusion In the diagnosis of thyroid nodules,color Doppler ultrasound can be combined with hemodynamic parameters to differentiate benign and malignant tumors,providing a reference for doctors to formulate surgical plans.
