二极管激光联合养龈固齿汤辅助治疗牙周牙髓联合病变的疗效研究

Clinical effect of Yangyin Guchi decoction and diode laser in the treatment of combined periodontal and endodontic lesions

目的探讨二极管激光辅助超声龈下刮治同步养龈固齿汤冲洗治疗牙周牙髓联合病变的临床疗效。方法选取在医院就诊的牙周牙髓联合病变患者122例,共138颗牙,采用随机数表法分为对照组(61例,72颗牙)、观察组(61例,66颗牙)。两组均行根管治疗和牙周基础治疗,对照组观察对象在超声龈下刮治的同时运用养龈固齿汤进行袋内冲洗治疗,观察组在此基础上进行二极管激光治疗。对比分析两组临床附着水平(CAL)、牙周袋的探诊深度(PD)、牙龈指数(GI)、改良出血指数(mBI)、根尖周指数(PAI)并进行疗效评价。结果治疗后观察两组患者,牙周学指标mBI、PD、CAL、GI及PAI指标等都有所改善(P<0.05),而且观察组对象的牙周治疗结果优于对照组,其差异有统计学意义(P<0.05);治疗后对照组总有效率为68.05%,观察组总有效率为89.39%,观察组临床疗效显著高于对照组(P<0.05)。结论超声龈下刮治联合养龈固齿汤及辅助二极管激光治疗,可有效改善牙周牙髓联合病变患者的临床症状,提高治疗效果。

Objective To explore the clinical curative effect of ultrasonic subgingival scaling with Yangyin Guchi decoction.Methods A total of 138 teeth(122 patients with periodontal pulp combined disease)were selected and divided into control group(61 cases,72 teeth)and observation group(61 cases,66 teeth)by random number table method.The patients in the control group were treated with ultrasonic subgingival scaling and Yangyin Guchi Decoction,while those in the observation group were treated with the addition of diode laser.The clinical attachment level(CAL),probing depth(PD),gingival index(GI),modified bleeding index(mBI),periapical index(PAI)were recorded and compared before and after treatment,the efficacy was assessed.Results After the treatment,the Index of mBI、PD、CAL、GI and PAI were improved(P<0.05),The observation group showed better improvement than the control group,and the difference was statistically significant(P<0.05).The total effective rate in the control group was 68.05%,and the total effective rate in the observation group was 89.39%.The clinical curative effect of the observation group was significantly higher than that of the control group(P<0.05).Conclusion Ultrasonic subgingival scaling with Yangyin Guchi decoction and diode laser in the treatment of combined periodontic-endodontic lesions can effectively improve the clinical symptoms of patients and enhance the therapeutic effect.
