不同程度宫颈病变中HPV感染特征分析

Analysis of HPV infection characteristics in different cervical lesions

目的:探讨不同程度宫颈病变中HPV感染特征分析,为宫颈病变防治提供参考依据。方法:选取2014~2018年到德阳市人民医院就诊的宫颈病变患者5582例,其中慢性宫颈炎3503例,低级别鳞状上皮内病变(LSIL)648例,高级别鳞状上皮内病变(HSIL)1079例,宫颈鳞状细胞癌352例,分析各组HPV的感染率、感染亚型及感染模式。结果:慢性宫颈炎组HPV感染率为24.66%,LSIL组HPV感染率为65.12%,HSIL组HPV感染率为87.03%,宫颈鳞状细胞癌组HPV感染率为91.67%,各组间阳性率差异有统计学意义(P<0.05);慢性宫颈炎组HPV感染率排名前五的亚型为52、16、58、53、81;LSIL组、HSIL组、宫颈鳞状细胞癌组HPV感染率排名前五的亚型为16、52、58、18、33;慢性宫颈炎组、LSIL组、HSIL组,宫颈鳞状细胞癌组均以单一感染为主,各组间单一感染率差异有统计学意义(P<0.05);宫颈鳞状细胞癌组中HPV各亚型感染模式差异有统计学意义(P<0.05)。结论:不同程度的宫颈病变中HPV感染特征有差异,HPV分型检测对于宫颈病变防治具有指导意义。

Objective:To explore the relationship between HPV infection characteristics and cervical lesions,and provide reference for the prevention and treatment of cervical lesions.Methods:A total of 5582 patients with cervical lesions who visited Deyang People's Hospital from 2014 to 2018 were selected,including3503 cases of chronic cervicitis,648 cases of LSIL,1079 cases of HSIL and 352 cases of cervical squamous cell carcinoma.Analyze the infection rate,infection subtype and infection pattern of HPV in each group.Results:The HPV infection rates in the chronic cervicitis group,LSIL group,HSIL group,and cervical squamous cell carcinoma group were 24.66%,65.12%,87.03%,91.67%,the difference in positive rate between groups is statistically significant(P<0.05).The top five HPV infection rates in the chronic cervicitis group are 52,16,58,53,81,and the top five HPV infection rates in the LSIL group,HSIL group,and cervical squamous cell carcinoma group are HPV 16,52,58,18,33.Chronic cervicitis group,LSIL group,HSIL group,cervical squamous cell group are mainly single infection,and the difference in single infection rate among the groups is statistically significant(P<0.05).There is a statistically significant difference in the single infection rate of HPV subtypes in the cervical squamous cell group(P<0.05).Conclusion:There are differences in the characteristics of HPV infection in different degrees of cervical lesions,and HPV typing detection has guiding significance for the prevention and treatment of cervical lesions.
