摘要
在老年慢性病高发的情况下,延续护理能够有效规范老年慢性病患者出院后的医疗行为,促进患者康复,从而降低患者再住院率。文章将我国老年慢性病患者延续护理的常见应用模式、开展方式、质量评价指标体系、存在的问题进行了综述,并提出建议,以期为制定适应于我国老年慢性病患者的延续护理模式提供参考依据。
Under the trend of high incidence of chronic diseases in the elderly,continuous nursing can effectively regulate the medical behavior of patients with chronic diseases after discharge,promote the rehabilitation of patients,and thus reduce the rate of re-hospitalization.In the paper,the application model,development mode,quality evaluation,existing problems and suggestions of continuing care for elderly patients with chronic diseases in China are reviewed.The aim is to provide basis for formulating a continuing care model suitable for elderly patients with chronic diseases in China.
