芒针深刺关元俞配合真武汤加味治疗寒湿型单侧腰椎间盘突出症临床研究

Clinical Study on Deep Needling with Elongated Needle at Guanyuan Combined with Supplemented True Warrior Decoction in the Treatment of Unilateral Lumbar Disc Herniation with Cold-Dampness Syndrome

目的:观察芒针深刺关元俞配合真武汤加味治疗寒湿型单侧腰椎间盘突出症的临床疗效。方法:选取2020年1月到2021年6月商丘市中医院针灸科治疗的寒湿型单侧腰椎间盘突出症患者100例作为研究对象,按照随机数字表法分为观察组和对照组,每组50例。对照组给予常规针刺配合真武汤加味治疗,观察组给予芒针深刺关元俞配合真武汤加味治疗。观察两组患者治疗前后视觉模拟评分(visual analogue score,VAS)、日本骨科学会制定的下腰痛评分(Japanese orthopaedic association scores,JOA)、抗人IgM免疫球蛋白、抗人IgG免疫球蛋白变化情况及临床疗效。结果:观察组有效率为96.00%,对照组有效率为86.00%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后VAS评分、JOA评分低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为4.00%,对照组不良反应发生率为2.00%,观察组不良反应发生率高于对照组,但差异无统计学意义(P>0.05);观察组治疗后抗人IgM免疫球蛋白、抗人IgG免疫球蛋白水平低于对照组,差异有统计学意义(P<0.05)。结论:芒针深刺关元俞配合真武汤加味治疗寒湿型单侧腰椎间盘突出症,能够提高临床疗效,减轻患者疼痛以及临床症状。

Objective:To observe the clinical efficacy of deep needling with elongated needle at Guanyuan(BL 26)combined with Supplemented True Warrior Decoction on unilateral lumbar disc herniation with cold-dampness syndrome.Methods:A total of 100 patients with unilateral lumbar disc herniation with cold-dampness syndrome treated in The Acupuncture and Moxibustion Department of Shangqiu Hospital of Traditional Chinese Medicine from January 2020 to June 2021were selected as the research objects,and divided into the observation group and the control group according to the random number table,with 50 cases in each group.The control group was treated with routine acupuncture combined with Supplemented True Warrior Decoction,while the observation group was treated with deep needling with elongated needle at Guanyuan(BL 26)combined with Supplemented True Warrior Decoction.The visual analog score(VAS),Japanese orthopaedic association scores(JOA),anti-human IgM immunoglobulin and anti-human IgG immunoglobulin changes and clinical efficacy of the two groups before and after treatment were observed.Results:The effective rate of the observation group was 96.00%,while that of the control group was 86.00%.The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS score and JOA score of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05);The incidence of adverse reactions in the observation group was 4.00%,while the incidence of adverse reactions in the control group was 2.00%.The incidence of adverse reactions in the observation group was higher than that in the control group,but the difference was not statistically significant(P>0.05);The levels of anti-human IgM immunoglobulin and anti-human IgG immunoglobulin in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Deep needling with elongated needle at Guanyuan(BL 26)combined with Supplemented True Warrior Decoction can improve clinical efficacy,alleviate pain and clinical symptoms in patients with unilateral lumbar disc herniation of cold-dampness type.
