血清omentin-1、vcam-1表达与肾移植术后心血管事件发生的相关性研究

Correlation Between Serum Omentin-1 and VCAM-1 Expression and Cardiovascular Events After Renal Transplantation

目的:探究血清网膜素-1(omentin-1)、血管细胞黏附分子-1(vcam-1)表达水平与肾移植术后心血管事件发生的相关性。方法:选取2020年12月—2021年12月于郑州市第七人民医院肾移植肾脏病诊疗中心接受肾移植手术的患者62例,根据术后心血管事件发生情况分为暴露组和非暴露组。收集患者一般临床资料和生化指标水平,ELISA法检测血清omentin-1、vcam-1水平,Pearson相关性分析法分析血清omentin-1、vcam-1水平与部分生化指标的相关性,受试者工作特征曲线(ROC)分析血清omentin-1、vcam-1表达对肾移植术后心血管事件发生的预测价值。结果:62例接受肾移植手术的患者中,共16例患者术后发生心血管事件,发生率为25.81%;暴露组患者的收缩压、血清N末端B型利钠肽原(NT-proBNP)、心肌钙蛋白Ⅰ(cTnⅠ)、24 h蛋白尿、尿素氮(BUN)、肌酐(Scr)、糖化血红蛋白(HbAlc)、空腹血糖(FPG)和胆固醇(TC)水平均明显高于非暴露组(P<0.05)。暴露组患者血清omentin-1水平低于非暴露组,vcam-1水平高于非暴露组,差异比较有统计学意义(P<0.05)。相关性分析结果显示,omentin-1水平与NT-proBNP、cTnⅠ、24 h蛋白尿、BUN、Scr、HbAlc、FPG和TC均呈明显负相关,vcam-1水平与以上生化指标呈明显正相关(P<0.05)。ROC曲线分析结果显示,血清omentin-1、vcam-1水平预测肾移植患者术后心血管事件发生的AUC值分别为0.760、0.847,差异均有统计学意义(P<0.05)。结论:血清omentin-1在肾移植术后发生心血管事件的患者中表达降低,vcam-1表达升高;二者表达水平与患者的心肌酶、肾功能和糖脂代谢相关,在预测肾移植术后心血管事件的发生方面具有一定的指导意义。

Objective:To investigate the correlation between the expression levels of serum omentin-1 and vascular cell adhesion molecule-1(VCAM-1)and cardiovascular events after renal transplantation.Methods:A total of 62 patients who underwent renal transplantation in the Department of Kidney Transplantation and Nephrology,the 7th People’s Hospital of Zhengzhou between December 2020 and December 2021 were selected.They were divided into exposure group and non-exposure group according to the occurrence of cardiovascular events after operation.The general clinical data and biochemical indicators of the patients were collected.Serum omentin-1 and VCAM-1 levels were detected by ELISA.Pearson correlation analysis was performed to analyze the correlation between serum omentin-1 and VCAM-1 levels and some biochemical indicators.The receiver operating characteristic(ROC)curve was used to analyze the value of serum omentin-1 and VCAM-1 in predicting cardiovascular events after renal transplantation.Results:A total of 16 patients developed cardiovascular events after operation,with an incidence of 25.81%.Systolic blood pressure,serum N-terminal prohormone of brain natriuretic peptide(NT-proBNP),cardiac troponin I(cTnI),24h urine protein,blood urea nitrogen(BUN),serum creatinine(Scr),glycosylated hemoglobin(HbAlc),fasting plasma glucose(FPG)and total cholesterol(TC)levels in the exposure group were significantly higher than those in the non-exposure group(P<0.05).Serum omentin-1 level in the exposure group was lower than that in the non-exposure group,and VCAM-1 level was higher than that in the non-exposure group(P<0.05).Correlation analysis results showed that omentin-1 level was significantly negatively correlated with NT-proBNP,cTnI,24 h urine protein,BUN,Scr,HbAlc,FPG and TC,while VCAM-1 level was significantly positively correlated with the above biochemical indicators(P<0.05).ROC curve analysis showed that the AUC values of serum omentin-1 and VCAM-1 levels to predict cardiovascular events after renal transplantation were 0.760 and 0.847,respectively(P<0.05).Conclusion:Serum omentin-1 level is decreased,and VCAM-1 level is increased in patients with cardiovascular events after renal transplantation.The expression levels of the two are related to myocardial enzymes,renal function,and glucose and lipid metabolism,which indicates that they can help to predict the occurrence of cardiovascular events after renal transplantation.
