晚期消化道恶性肿瘤患者安宁疗护需求及与家属认知程度 社会支持情况的相关性

Correlation among hospice care need, family perception level and social support in patients with late malignant tumor of digestive tract

目的探讨晚期消化道恶性肿瘤患者安宁疗护需求及与家属认知程度、社会支持情况的相关性。方法将125例消化道恶性肿瘤患者作为研究对象,采用自制安宁疗护需求量表、家属认知程度量表及社会支持情况量表进行调查,采用单因素分析及Logistic回归分析探讨消化道恶性肿瘤患者安宁疗护需求影响因素,利用乘法及加法模型评定家属认知程度、社会支持情况交互作用对安宁疗护需求的影响。结果入组患者安宁疗护需求评分为(60.61±4.79)分;年龄、人均月收入、文化程度、家属认知程度及社会支持均为消化道恶性肿瘤患者安宁疗护需求的影响因素(P<0.01);消化道恶性肿瘤患者家属认知程度、社会支持情况与安宁疗护需求呈显著正相关(P<0.01);家属认知程度单独所致的比值比为2.125,社会支持单独所致的比值比为1.607,两者同时存在时,交互作用比值比为6.429,相关系数为3.918,提示家属认知程度、社会支持情况在消化道恶性肿瘤患者安宁疗护需求呈正向交互作用,6.429>2.125×1.607提示家属认知程度与社会支持情况为超相乘模型。结论消化道恶性肿瘤患者安宁疗护需求处于中等偏上水平,与家属认知程度、社会支持情况呈显著正相关,临床应予以充分重视。

Objective To explore the correlation among hospice care needs,family perception level and social support in patients with late malignant tumor of digestive tract(MTDT).Methods Investigations were conducted in 125 patients with late MTDT using the Self-made Hospice Care Needs Scale(HCNS),Family Perception Care Scale(FPCS)and Social Support Rating Scale(SSRS),factors influencing hospice care needs analyzed using single-and multi-factor manner,and the effects of family perception level and social support situation interaction on hospice needs evaluated using multiplicative and additive models.Results The average score of hospice care needs for enrolled patients was(60.61±4.79);factors influencing patients’hospice care needs were age,per capita monthly income,educational level,family perception level and social support(P<0.01);family perception level and social support were significantly positively correlated to hospice care needs(P<0.01);the odds ratio of family cognitive level alone was 2.125,social support 1.607,when coexistence of both the interaction OR 6.429,correlation coefficient 3.918,which suggested that family cognitive level and social support had a positive interaction effect on hospice care needs,and 6.429>2.125×1.607 did that family perception level and social support were supermultiplication model.Conclusion Hospice care needs in MTDT patients are above the medium level,are significantly positively correlated with family cognitive level and social support,and to which high attention should be paid clinically.
