探讨关节镜手术治疗膝关节半月板损伤的临床疗效

Exploring the Clinical Efficacy of Arthroscopic Surgery for Knee Meniscal Injury

目的探讨关节镜手术治疗膝关节半月板损伤的临床疗效。方法随机选择2020年1月—2022年6月三明永安总医院收治的诊断为膝关节半月板损伤的80例患者,以随机数表法分成两组,每组40例,实施开放性手术治疗为对照组,接受关节镜手术治疗为研究组。比较两组患者治疗前后的屈膝90°及屈膝30°前向松弛情况、膝关节功能评分、膝关节活动度,统计围术期相关指标、治疗后膝关节恢复情况并发症及发生情况。结果治疗后,两组屈膝90°及屈膝30°前向松弛情况、膝关节功能评分、膝关节活动度较治疗前有所提升,研究组提升程度更优于对照组,差异有统计学意义(P<0.05)。相较于对照组,研究组的手术时间和术后首次下床时间更短,术中出血量更少,术后疼痛感更小,差异有统计学意义(P<0.05)。研究组膝关节功能恢复优良率(97.50%)高于对照组(80.00%),而术后并发症率(5.00%)低于对照组的22.50%,差异有统计学意义(χ^(2)=4.507、5.165,P<0.05)。结论膝关节半月板损伤以关节镜手术,可缩短手术与术后首次下床时间,减少术中出血量与术后的并发症,降低术后疼痛感,有效改善治疗后的屈膝90°及屈膝30°前向松弛情况,提升膝关节功能及其活动度。

Objective To observe and discuss the clinical efficacy analysis of arthroscopic surgery for knee meniscus injury.Methods From January 2020 to June 2022,80 patients diagnosed with meniscus injury of knee joint in Sanming Yong'an General Hospital were randomly selected and divided into two groups by random number table method,40 cases in each group.Among them,open surgery was used as the control group,while arthroscopic surgery was used as the study group.The knee flexion 90°and knee flexion 30°forward relaxation,knee function score and knee joint activity were compared between the two groups before and after treatment.Statistical analyed of perioperative related indicators,knee joint recovery after treatment,and incidence of complications.Results After treatment,the anterior relaxation at 90°and 30°flexion,knee joint function score,and knee joint range of motion of the two groups were improved compared to before treatment,the improvement degree of the research group was better than that of the control group,and the difference was statistically significant(P<0.05).Compared to the control group,the study group had shorter surgical time and first postoperative bedridden time,less intraoperative bleeding,and less postoperative pain,and the difference was statistically significant(P<0.05).The excellent and good recovery rate of knee joint function in the study group was 97.50%higher than(80.00%)in the control group,while the postoperative complication rate was 5.00%lower than(22.50%)in the control group,and the difference was statistically significant(χ^(2)=4.507,5.165,P<0.05).Conclusion Arthroscopic surgery for meniscus injury of the knee joint can shorten the time between surgery and first postoperative bedtime,reduce intraoperative bleeding and postoperative complications,reduce postoperative pain,effectively improve the anterior relaxation of knee flexion at 90°and 30°after treatment,and enhance knee joint function and mobility.
