AMI患者心电图QRS波振幅间期乘积与梗死面积、PCI术后并发MACE的关系

Relationship among infarct size, complicated MACE after PCI and ECG QRS wave amplitude interval product in AMI patients

目的:探讨急性心肌梗死(AMI)患者心电图QRS波振幅间期乘积与梗死面积、经皮冠脉介入治疗(PCI)术后并发主要不良心血管事件(MACE)的关系。方法:选择我院收治的243例AMI患者,均于入院后进行心电图检查,采用Pearson法分析心电图QRS波振幅间期乘积和梗死面积的相关性;所有患者PCI术后随访6个月,根据MACE的发生情况分为MACE组(51例)和无MACE组(189例),对比两组一般资料及心电图QRS波振幅间期乘积,采用Logistic多元回归法分析PCI术后并发MACE的危险因素。结果:Pearson分析显示,QRS波振幅间期乘积与梗死面积呈显著正相关(r=0.743,P=0.001)。Logistic多元回归分析显示前壁心肌梗死、Killip分级Ⅲ~Ⅳ级、QRS波振幅间期乘积均为MACE发生的独立危险因素(OR=1.154~2.585,P均<0.05)。结论:AMI患者心电图QRS波振幅间期乘积对判断梗死面积及评估PCI术后MACE的发生风险有重要意义。

Objective:To explore relationship among infarct size,complicated major adverse cardiovascular events(MACE)after percutaneous coronary intervention(PCI)and ECG QRS wave amplitude interval product in patients with acute myocardial infarction(AMI).Methods:A total of 243 AMI patients treated in our hospital received ECG examination after hospitalization.Pearson method was used to analyze the correlation between ECG QRS wave amplitude interval product and infarct size;all patients were followed up for six months after PCI,they were divided into MACE group(n=51)and no MACE group(n=189)according to presence of MACE.General data and ECG QRS wave amplitude interval product were compared between two groups.Multivariate Logistic regression was used to analyze risk factors for complicated MACE after PCI.Results:Pearson correlation analysis indicated that QRS wave amplitude interval product was significant positively correlated with infarct size(r=0.743,P=0.001).Multivariate Logistic regression analysis indicated that anterior wall myocardial infarction,Killip classⅢ~Ⅳand QRS wave amplitude interval product were independent risk factors for MACE in AMI patients after PCI(OR=1.154~2.585,P<0.05 all).Conclusion:ECG QRS amplitude interval product is of great significance for judging infarct size and assessing risk of MACE after PCI in AMI patients.
