基于SS-OCTA视网膜及脉络膜形态结构研究的湿性年龄相关性黄斑变性患者

摘要 目的应用超广角扫频源光学相干断层扫描血管成像(SS-OCTA)观察湿性年龄相关性黄斑变性(AMD)患者视网膜及脉络膜的形态结构特征。方法本研究为回顾性横断面研究。选取2020年9月~2021年12月成都中医大银海眼科医院收治确诊为湿性AMD的患者20例35只眼(病例组),并选取同期体检正常健康者13例26只眼(对照组)纳入本研究。使用SS-OCTA测量两组黄斑中心凹下视网膜厚度(SFRT)、黄斑区视网膜内层血管密度、黄斑区视网膜浅层复合体小血管密度及黄斑中心凹下脉络膜厚度(SFCT)、脉络膜血管指数(CVI)的差异。结果与对照组相比,病例组SFRT明显增加,差异有统计学意义(P<0.05),黄斑区视网膜内层血管密度及浅层血管复合体小血管密度明显降低,差异均有统计学意义(P<0.05);SFCT及CVI无明显变化,差异均无统计学意义(P>0.05)。结论与正常人相比,湿性AMD患者SFRT厚度增加、黄斑区视网膜内层血管密度及浅层血管复合体小血管密度减少,SFCT及CVI无明显变化。SS-OCTA对AMD的进一步诊断、监测具有重要意义。 Objective To observe retinal and choroidal morphological structure in patients with wet age-related macular degeneration(AMD)by using swept-source optical coherence tomography angiography(SS-OCTA).Methods This study was a retrospective cross-sectional study.From September 2020 to December 2021,20 cases(35 eyes)with wet AMD were retrospectively selected from Chengdu Traditional Chinese Medicine Ineye Hospital(case group),and 13 cases(26 eyes)with normal physical examination in the same period(control group)were included in this study.The differences of subfoveal retinal thickness(SFRT),vascular density of the inner retina,small vascular density of the superficial vascular,subfoveal choroidal thickness(SFCT)and choroidal vascularity index(CVI)between the two groups were compared by SS-OCTA.Results SFRT of the case group was significantly increased compared with the control group,and the difference was statistically significant(P<0.05).The vascular density of the inner retina and small vascular density of the superficial vascular complex were significantly reduced,and the difference was statistically significant(P<0.05).There was no significant change in SFCT and CVI,and the difference was not statistically significant(P>0.05).Conclusion Compared with normal people,wet AMD patients have increased SFRT thickness,decreased vascular density in the inner retina and small vascular density in the superficial vascular complex,there were no significant differences in SFCT and CVI.SS-OCTA have great significance for the further diagnosis,monitoring of AMD.
